Bilingual (Spanish/English) Outreach and Patient Care Coordinator,
Bellevue Hospital Center, Parkinson and Related Movement Disorders
Clinic (75% FTE)
Seeking experienced Social Worker (LCSW) position to develop community
outreach and patient care at Bellevue Hospital Parkinson and Related Movement Disorders Clinic. Bellevue is the flagship facility of New York City's Health and Hospitals Corporation and is affiliated with New York University. More than 80 percent of Bellevue's patients come from the city's medically underserved populations. Opportunity to develop and shape new programs. Will interface with a multidisciplinary clinical staff and multicultural patient population . Seeking mature individual who works well both independently and with teams. No experience with Parkinson disease required.
Specific duties include:
Coordination of the center's outreach activities in collaboration
with Bellevue Outreach service
Identifying and partnering with community agencies such as senior centers, nursing homes and community organizations to increase awareness of illness and resources for people with Parkinson Disease Performance of patient and caregiver psychosocial assessments and some short term counseling


Serve as a first contact/facilitator for patients seeking care in the Parkinson clinic.
Initiation of patient and caregiver support groups

Assist patients and their families with obtaining community resources
