FIELD TRIP PERMISSION SLIP
The Elementary Orchestra students will be traveling to Bellefonte High School after school on Monday, May 16th to rehearse for our Spring Concert. Rehearsal will run from 3:30-5:30 in the HS Theatre and students are to be picked up from the theatre at 5:30 pm. The concert will also be held in the in the Bellefonte High School Theatre on Tuesday, May 17th beginning at 7:00. Please return the signed permission slip to your child’s teacher by May 6th.

I hereby grant permission for my child named above to participate in the
Orchestra Field trip to Bellefonte High School from 2:40-5:30 pm on Monday, May 16th
I understand that the transportation to the High School will be by Bus and that Ms. Silvis will be in charge. I also understand that I will need to pick my child up in the High School Theatre at 5:30 P.M.
In the event of an accident or medical emergency and I am unavailable for the purpose of providing parental consent, I hereby authorize the physician(s) and staff in the Emergency /Outpatient Department of the appropriate hospital to provide such hospital care that includes routine diagnostic procedures and medical treatment as necessary to my minor son/daughter. I understand that the authorization herein granted does not include MAJOR surgical procedures.

Below is information about our Spring Concert and rehearsal. It is amazing how fast the end of the year is coming and how few rehearsals we have left until the concert! Please be sure to sign and return the attached permission slip ASAP. Feel free to call or e-mail me if you have any questions.

Spring Concert Information
After School Rehearsal
On Monday, May 16th, all Band, Orchestra and Choir Students will be traveling after school to Bellefonte High School to rehearse for our Spring Concert. They will be riding after school busses like they did for the Gala concert so they will not miss any classes. The rehearsal will run from 3:30-5:30 for band and orchestra students. Students who are ONLY in choir may be picked up at 4:30. All Orchestra and Band students must be picked up by a parent or guardian inside of the theatre at 5:30. If your child cannot attend the rehearsal, please contact their director prior to the rehearsal! Please sign and return the attached permission slip to your teacher by May 6th.
Spring Concert Information

Where: Bellefonte High School Theatre
Report Time: 6:30 pm Performances begin: 7:00 pm
Dress: Please dress your best.
Try to avoid sweats, jeans, sneakers and graphic tees!
Who performs: Elementary Band and Orchestra Students and 5th Grade Choir from Bellefonte and Benner Elementary Schools
This concert is required and students do receive a grade for their performance. Our ensembles work together as a team to perform. If we are missing one person we are missing a very important part of our team! If your child cannot attend please inform their director as soon as possible. If an emergency occurs, please contact your child’s director within 24 hours of the concert!

***Mark your calendars: Summer Orchestra and Band Camp will be held from 10:00 A.M. – Noon weekdays from July 11th – July 22nd at the Middle School. Students are encouraged to come to as many days as they can to play with students from all schools in the district. More information and registration forms coming soon.
