If parents or godparents HAVE attended a baptism seminar within the past six years, a copy of their attendance certificate is required. 1) Baptism Seminar Certificate for parents and godparents 2) Certificate of Baptism with notations is required of the Godparent(s) 3) Godparent eligibility form 4) priests letter must be turned in prior to the baptism being scheduled with the priest or deacon. *Christian Witnesses are not required to submit any paperwork. Note: Currently Baptisms take place within the context of either the Vigil (4:00PM), 9:30AM, or Noon Mass.
