CUSTOMER INFORMATION

SIN 132-51 / Information Technology Professional Services

1a. Table of awarded special item number(s):

The enclosed price list contains Contractor site prices for each labor category for each year. Contractor-site rates are listed by Line Item Number for each Labor Category that ends in “C”. Government-site labor may be directly requested using the same service labor categories and descriptions by using Line Item numbers ending in “G” at the order level when necessary to meet task order requirements.

1b. Identification of the lowest priced model number and lowest unit price for that model for each special item number awarded in the contract. The prices shown are the lowest price per hour based on a unit of one hour, exclusive of any quantity/dollar volume, prompt payment, or any other concession affecting price.

1c. The Contractor is proposing hourly rates, a description of all corresponding commercial job titles, experience, functional responsibility and education for those types of employees or subcontractors who will perform services is provided beginning on page 4.

2.  Maximum order. $500,000.00

3.  Minimum order. $100.00

4.  Geographic coverage (delivery area). Prices are valid for CONUS, 48 contiguous states including District of Columbia

5.  Point(s) of production (city, county, and State or foreign country). INNOVA sells Labor hours. Labor hours will be performed in INNOVA facilities (Contractor Site) unless purchased for use in Government facilities based on task order requirements.

6.  Discount from list prices or statement of net price. The prices shown represent the GSA Client’s net price per hour per labor category and includes 0.75% Industrial Funding Fee (IFF).

7.  Quantity discounts. Orders over $500,000.00 will receive a 0.5% (one half of one percent) discount on the price before IFF.

8.  Prompt payment terms. Terms are NET 30. Information for Ordering Offices: Prompt payment terms cannot be negotiated out of the contractual agreement in exchange for other concessions.

9a. Government purchase cards are accepted at or below the micro-purchase threshold.

9b. Government purchase cards are not accepted above the micro-purchase threshold.

10. Foreign items (list items by country of origin). No Foreign items are offered.

11a. Time of delivery. Services are delivered on Government site within 30 days and are available at contractor site within 21 days unless otherwise specified in individual task orders.

11b. Expedited Delivery. All items in the price list are available for expedited delivery. Please contact the contractor for specifics.

11c. Overnight and 2-day delivery. Services delivered at the contractor site may be expedited using virtual workforce technologies including “Go To Meeting”, SharePoint document delivery, teleconference and other forms of virtual collaboration to speed understanding and delivery. INNOVA specializes in the on-demand economics and expedited value of services delivered via the world-wide web. Please contact the contractor for your specific needs.

11d. Urgent Requirements. The Contractor will note in its price list the “Urgent Requirements” clause of its contract that allows for expedited delivery and availability of resources at the contractor’s site by contacting the contractor directly at the above address to achieve a faster delivery. INNOVA Service Delivery is designed to meet urgent requirements through use of existing staff, consultants, and part time, on-demand workforce to deliver services rapidly via collaboration with our clients using the worldwide web. We also use a network of recruiters to quickly supplement our workforce or deploy additional, skilled labor to a client’s site. 12. F.O.B. point(s). All services are available at INNOVA’s offices in the Kansas City Metro Area, Omaha NE, Atlanta GA, and Scottsdale AZ. Skill Level Definitions table - Most Items are available in any of 3 skill levels based on years of experience, depth of understanding and application in the field, and level of supervision or leadership.

Labor Categories with Pricing Table – provides item numbers and pricing for each labor category and skill level offered. Level I less than 5 years’ experience / Requires a HS Diploma (BA/BS Preferred) and 0-5 years of experience in the field or in a related area. Applies expertise to Government and commercial common user systems, as well as to dedicated special purpose systems requiring specialized security features and procedures.
