Authorization for Use of Physical Image on HUD Social Media

I (print name)______, hereby authorize and consent to the use of my image and audio by the U.S. Department of Housing and Urban Development to be posted publically on HUD’s social media platforms. This includes the Department’s official blog: The HUDdle, HUD’s YouTube channel, and HUD’s Facebook, Twitter, Flickr and Instagram accounts. This also includes internal and external HUD electronic, print publication and websites.

This authorization is irrevocable, and waives all rights, claims, and demands against the federal government in connection with or arising out of the reproduction, publication or other use of photograph or other physical images of me.

I acknowledge that my participation is voluntary and that I will receive no financial compensation now or in the future.
