Authority to Publish

This form is an agreement between you, the University of Technology, Sydney (‘UTS’) and the NSW Department of Education and Training. Please read it carefully before signing it below.

UTS and the NSW Department of Education and Training would like to use photographs and/or video images taken of you during the Ngana Mai: Dream U@UTS Day event on Thursday 16 May (‘Images’), and/or spoken or written work or any other works produced by you at the Ngana Mai: Dream U@UTS Day event (‘Materials’) to promote and publicise public education. The use of the Images and Materials may include, but is not limited to, reproduction throughout the world in electronic and print promotional material, websites and in media promotions.

By signing this form you agree to the following:

1.   UTS and the NSW Department of Education and Training are entitled to use the Images and Materials, as many times and in as many ways as required, without any payment to you.

2.   The Images and Materials may be reproduced in colour or black and white and may be adapted, re-sized, altered, distorted and/or blurred for design purposes.

3.   You will not be consulted about the specific context or manner in which the Images and/or Materials are published.

4.   You consent to use of the Materials without being identified as the author of the Materials.

UTS and the NSW Department of Education and Training will not use the Images and Materials for any purpose other than the promotion of public education. Should you wish to restrict the use of the Images and Materials further, please provide details at the bottom of this form.

Your agreement to permit the use of the Images and Materials is greatly appreciated. Any inquiries you have may be directed to Jumbunna Indigenous House of Learning on (02) 9514 1902.

If you are under 18 years of age we will need the written agreement of you and your parent/guardian before UTS and NSW Department of Education and Training can use the Images or Materials. Please have them sign this form.
