Dear Mr. Zarza:

I am a monthly pass holder for the San Bernardino line between San Bernardino and Union Station.

On March 21, 2008, I intended to ride the 312 / 314 train between Union Station and San Bernardino. The train broke in west of the Montclair station and I sat on the train for more than an hour before the 314 train arrived and pushed it to Montclair. I finally arrived at my destination more than one hour after the scheduled arrival time for my train.

The delay was more than one hour.

Attached is a photocopy of my signed monthly pass.

In accordance with Metrolink’s Quality Service Pledge, I am requesting a Universal 4-trip ticket.

Whether or not you intend to use the 4-trip ticket, please make Metrolink go through the process of sending you the ticket. You may give the ticket to someone who can use it or you can use it on any Metrolink train on a weekend where your monthly pass would not cover the fare.

Making no effort says to Metrolink that you accept their failure to provide safe train service along their routes.

II. Extensive Delay - One-Time Incident

An extensive delay is a delay to riders exceeding one hour from the published train schedule.

Monthly Pass Holder
Upon request, with proof of pass ownership, a monthly pass holder is eligible to receive a Universal 4-trip ticket with a one-year expiration date from the date of the incident.

One-way, Round-trip or 10-trip Ticke Holder
One way, round-trip or 10-trip ticket holder receives a replacement ticket no less than the value of the trip that was extensively delayed.
