REGISTRATION FOR SAY’S EXTENDED DAY

PROGRAM FOR THE 2016/2017 SCHOOL YEAR

The Extended Day Program offers high-quality on-site child care after school. The program is licensed by the State of California and maintains a ratio of at least one teacher for every 14 children. The program is a fun and rewarding experience for your child, providing a wide range of activities especially designed with your child’s age, skills, and interests in mind.

Enrollment is open to all students attending Kumeyaay Elementary. Fees are established to cover the cost of the program. There is a 20% discount for additional siblings from the same family.

To register for the Extended Day Program: (Limited space available. Registration is on a first come, first served basis).

1. Complete the registration form below and attach next year’s annual $50 registration processing fee. The annual fee is required for each child and is non-refundable. The fee covers the processing of your registration and will not be refunded.
2. Mail this registration form and registration fee to: SAY, 8755 Aero Drive, Suite 100, San Diego, CA 92123. Registration will be on a first come first served basis. If the program is full we will place you on a waitlist. You will be called to enroll as space becomes available. Your $50 registration fee will hold your place on the wait list.
3. An enrollment packet and fee schedule will be mailed to those in the initial group to be enrolled no later than June 27, 2016. The packet must be completed and returned, with fees for August/September by July 29, 2016 to complete your enrollment.

ENROLLMENT IS NOT COMPLETE UNTIL ALL ENROLLMENT FORMS ARE COMPLETE AND AUGUST/SEPTEMBER 2016 FEES ARE PAID.
