A. ORGANIZATIONAL OVERVIEW – 10 Points

b. All staff in charge of facilitating any of the proposed evidence based training or skills building groups described in RFP Section 2.9, must be trained by a certified trainer prior to running groups. B. WORK PLAN – 60 Points
Collaborations or joint submissions are permitted. For agencies submitting joint applications, the lead agency must be the agency with the expertise in behavioral health services and be able to demonstrate prior experience as prescribed below in the Minimum Qualifications.
Agencies submitting joint applications as a collaborative must clearly articulate the role of the lead applicant in each of the questions below, describe the distribution of labor, and responsibilities across all collaborative partners.
1. CASC Service Model:
a. (10 Points) Describe agency’s experience managing a Forensic Assertive Community Treatment team and how does the agency propose to implement the FACT team at the CASC? Include agency’s experience working with clients who :
i. Have severe mental illness, which may include the following diagnoses: co-occurring disorders, schizophrenia, bipolar disorder, personality disorders, general anxiety/mood disorders, and PTSD.
ii. Have come into contact with the criminal justice system and have repeated incarcerations.
iii. Have substance use/dependency issues.
iv. May be homeless or experience multiple barriers to housing

SFAPD does not anticipate any capital expenses associated with this project.
Budget proposal may include indirect rates no higher than 12% of the entire contract’s costs which includes Salaries, Benefits, and Operating Expenses.
