Minimum Requirements ACKNOWLEDGEMENT form

INSTRUCTIONS: Vendors must affirm the following Minimum Requirements as applicable to the Service Categories offered in their Reply.A failure to respond to a Service Category will be treated as “not applicable.” Otherwise, failure to submit a response for any Service Category, or selection of the response “No” will disqualify the Vendor from further consideration for that Service Category.

  1. For Office Building Space Vendors. If proposing to provide Office Building Space (see ITN Section 2.3(A)), Vendor must (i) currently have available and agree to provide at least 150 workspaces within 50 miles of downtown Jacksonville, FL, (ii) agree to provide one personal computer per workspace as defined in Section 2.3(A) of the ITN, and (iii) be able to provide the Services on or before June 15, 2018. Does the Vendor’s Reply meet these requirements?

  1. For Mobile Recovery Unit Vendors. If proposing to provide Mobile Recovery Units (see ITN Section 2.3(B)), Vendor must (i) be able to deliver and setup workspaces for at least 350 employees anywhere within the State of Florida, (ii) agree to provide one personal computer per workspace as defined in Section 2.3(B) of the ITN, and (iii) be able to provide the Services on or before June 15, 2018. Does the Vendor’s Reply meet these requirements?

  1. For Personal Computer Vendors. If proposing to provide Personal Computers (see ITN Section 2.3(C)), Vendor must be able to (i) deliver no less than 325Personal Computers to specified addresses in Florida within 24 hours of request, (ii) agree to provide Personal Computers meeting the specifications in 2.3(C) of the ITN, and (iii) be able to provide the Services on or before June 15, 2018. Does the Vendor’s Reply meet these requirements?

  1. For On-Demand Satellite Communications Vendors. If proposing to provide On-Demand Satellite Communications (see ITN Section 2.3(D)), Vendor must (i) be able to deploy and make available within 48 hours, anywhere in the State of Florida, a Satellite Communication solution meeting the specifications defined in Section 2.3(D) of the ITN, and (ii) be able to provide the Services on or before June 15, 2018. Does the Vendor’s Reply meet these requirements?
