Athlete and Unified Partners

Due on March23rd

Athletes-must have an active medical by the March 23rd deadline.

For any brand new Athletes, you must have the doctor signed medical submitted as well with the paperwork on March 23rd. If the athlete Medical expires between March 23rd and May 18th (the start of the games) they will need to get an updated Medical submitted to SOGA prior to the expiration date of the Medical.

PLEASE FAX YOUR MEDICAL FORM STRAIGHT TO THE SOGA OFFICE. THE FAX NUMBER IS ON THE MEDICAL FORM. Keep a copy in your file.

Unified Partners that are planning to compete-

Must have aVOLUNTEER & PARTNER PROFILE FORM-on file or one must be submitted along with the Paperwork on March 23rd in order for them to compete.

NEW Unified Partners under the age of 18 by May 18th, 2018 must fill out the attached newVOLUNTEER & PARTNER PROFILE FORM.They will only need to fill out the first half of the page. They are not required to put in a photo ID Drivers license, as most will not have it. They will need to signand have parent sign and date bottom of the last page.Please send directly to SOGA. Keep a copy in your file.

EXISTING Unified Partners under the age of 18 by May 18, 2018(volunteers who have previously filled out the OLD "Unified Sports Partner Application") do not need to do anything further as long as they have previously sent in this oldform to SOGA.

Any one turning 18 before May 18, 2018 will need to fill out the new form and fulfill all the requirements of background check and remaining paperwork. Form must be in by deadline, but rest of paperwork can be done in a couple weeks if needed.

PROBLEM SHEET DEADLINE

APRIL 27TH

Unified Partners have until the 27th to complete and have the following submitted to SOGA.

*Coaches and/or Unified Partners that need to be screened

*Complete protective behaviors

*Submit the coach code of conduct form and/or complete the concussion training

Site Coordinators and Coaches will need to get confirmation on the following to Paula no later than April 23th (To give Paula time to get it updated in the SOGA system no later than April 27th)

*Changes, updates or initial-

-Level (I, II, III, V)

-Rating

-Singles play (add or delete)

-Unified doubles play (add, delete, unified partner assignment)

-Traditional doubles (add, delete, partner assignment)
