Specific Detailsof Incident, Observations, Behaviors of Associate: Due to falling below expectations of her Development Plan, Susie was placed on a Wireless Lifestyle Improvement Plan on 4/19/2016 with 2 week follow-ups to allow adequate time for improvement. On 5/3/2016, her Store Manager administered the first follow-up. Susie fell below expectations. On 5/27/16, the 2nd Follow-up has been administered; Susie continues to fall below expectations. Susie has failed to move out of the 4th and 5th quintile.
Impact of Incident/Behavior:By falling below expectations in her CSP metrics, Susie is trending to fail her WLIP. Since the WLIP implementation, Susie has remained in the 5th quintile with no improvement.
Behavior Expected of Associate:Susie must give her full commitment to meet or exceed the expectations listed in herWLIP.(For full list of expectations, see WLIP implemented on 4/19/2016). Susie must not fall below expectations for her Final follow-up. Failure to meet or exceed the expectations of her WLIP may result in further disciplinary action up to and including termination of employment.
Further violations of company policy may result in further disciplinary action up to and including termination.

Performance Improvement Plan

Measurable Improvement Goals/Expectations: Susie must be in the 3rd quintile or better in order meet expectations. Susie must meet all expectations listed in the WLIP by her Final follow-up. Should Susie fail to meet at least the 3rd quintile in performance, as well as meet all the expectations listed in the WLIP, disciplinary action up to and including termination of employment may occur.
Training or Special Direction to be Provided: Susie, her Store Manager and her District Manager have reviewed the WLIP for steps Susie must take to meet or exceed the expectations of her WLIP.Susie has been given the opportunity to discuss any questions she may have in regards to her 2nd follow-up with her Store and District Manager.
