Requirements:

Promotion from Assistant to Associate; no time limit

Portfolios may be submitted in paper form but must be appropriately organized in PDF with bookmarks.

Portfolio should demonstrate the expectation of being active in all of the domains of active study.

Select the domains in which the candidate is active.

Basic Science Candidate – first three must be checked OR Clinician Candidate – all four must be checked

Teaching Scholarship Service Clinical care (if relevant)

AND demonstrate progress in 2 of the 4 domains. Select the two in which the candidate is demonstrating progress.

Basic Science Candidate - pick 2, one of which must be scholarship OR Clinician Candidate - pick any two

Teaching Scholarship Service Clinical care (if relevant)

Must be completed:

Letter from Candidate specifying requested track/rank

Letter from Department Chair (see guidelines for required content)

Letter from Department APRT Committee (see guidelines for required content)

Updated CV in VTCSOM Format (from Digital Measures)

Support Letters:

Candidate furnishes up to 3 names to APRT Committee

APRT Committee selects names; ensures that at least one/no more than 2 are from those provided by candidate

If promotion from Assistant to Associate, 2 letters required (one must be from outside of Carilion and VT)

Letters should explicitly state whether candidate would meet requirements at VTCSOM

Letters must be from persons at the rank desired or higher

Publications:

Clinician candidate, from Assistant to Associate; no minimum number of publications

Basic science candidate, from Assistant to Associate; no minimum number of publications
