ARUP BLOOD SERVICES BIG GAME WINTER PROMOTION OFFICIAL CONTEST RULES 2008-2009
1. Dates/Eligibility: The ARUP Blood Services “Big Game giveaway” begins on or about November 24, 2008 and ends on or about January 24, 2009. ARUP Blood Services may change the dates and/or terms of the Contest without prior notice. Any changes to the contest will be posted on the web site www.utahblood.org. Each presenting donor of legal age, as determined by the Food and Drug Administration donor eligibility requirements, will be automatically entered in to the contest. The frequency of entry is limited to the blood donation eligibility requirements as defined by the Food and Drug Administration. No purchase or donation is necessary to enter. The giveaway is void where prohibited by law. Persons who have won a prize from ARUP Blood Services valued at $500 or more in the (12) month period prior to the start of the contest are not eligible to win.
2. How to enter: Each presenting whole blood donor to ARUP Blood Services from November 24, 2008 to January 24, 2009 will be entered automatically. Participants may donate blood at a mobile blood drive, ARUP Research site or ARUP Murray satellite location. Each presenting platelet donor to ARUP Blood Services will receive two entries per donation. Platelet donors may donate only at ARUP Research site or ARUP Murray satellite location. Each participating donor will automatically be entered by giving their information to ARUP Blood Services to complete the donation process and entered in to ARUP Blood Services records. Any one giving false or incorrect information will be disqualified. Those choosing not to donate and enter the contest can fill out an entry form at the reception desk at the ARUP Research Park location, ARUP Murray Satellite location or by asking a ARUP phlebotomy lead on a mobile blood drive for an entry card. The contest entities are not responsible for electronic, human, or computer error, late, misdirected or incomplete entries. By participating, entrants agree to follow these official rules. Entrants agree to decisions of the official ARUP Blood Services administration whose decisions are final and binding in all respects.
3. Bonus Entries: Whole blood donors who present to ARUP Blood Services between December 22, 2008 and January 9, 2009 will automatically be entered to receive three entries in to the contest. Platelet donors who present to ARUP Blood Services between December 22, 2008 and January 9, 2009 will automatically be entered to receive four entries in to the contest.
4. Selection of Grand Prize Winner: One grand prize winner will be selected on or about January 25, 2009 from the entries collected by ARUP Blood Services. If a grand prize winner cannot be located or contacted by ARUP Blood Services after a period of one month, ARUP Blood Services reserves the right to select an alternate winner and recipient.
5. Prizes: The grand prize winner will receive a flat screen TV, a ClearPlay DVD player, and three year ClearPlay membership. The second place prize winner will receive a Nintendo Wii. The winners will be responsible to make efforts to receive the prize from ARUP Blood Services and is responsible for the prize from that time forth. All prize terms and conditions will be fulfilled in winner’s own personal name. There will be no substitution of cash or alternate prizes for any contest winner.
6. Conditions/Restrictions: In exchange for the right to enter the ARUP Blood Services Big Game contest, each participant agrees to release and indemnify ARUP Blood Services and the sponsors and their respective officers, directors, agents, parent companies, subsidiaries and employees from all claims, demands and causes of any kind or nature whatsoever, whether known or unknown, foreseen or unforeseen that arise from or relate to the participant’s participation in the contest. Entry in to the contest constitutes permission for ARUP Blood Services and sponsors to use the participant’s name, likeness, portrait, picture, voice and or biographical information and material for an unlimited period of time in order to advertise, promote or publicize ARUP Blood Services and sponsors of the contest. As a condition of receipt of their prize, each winner will be required to produce identification satisfactory to ARUP Blood Services and sign an affidavit of eligibility and release (including publicity release) as prepared by ARUP Blood Services. Anyone using fraudulent means to participate in and or win the contest will be subject to disqualification. Failure to comply with official rules may result in disqualification.
7. Rules: Copies of these rules are free and available at ARUP Blood Services at 500 Chipeta Way, Salt Lake City, UT 84108 or at ARUP Blood Services Satellite center located at 5201 South Green Street, Murray, UT 84123 during regular business hours or by sending a request with a self-addressed stamped envelope to ARUP Blood Services at 500 Chipeta Way, Salt Lake City, UT 84108.
