I am glad to inform you that in continuation of promotion of Karate Do Sport, We are going to organize the 5th All India Independence Cup Karate Championship -2009on 4th and 5th October, 2009 (Sunday & Monday) at Bapu Samaj Auditorium, New Delhi.
Please find enclosed the attached events categories and entry form with this letter.
* The last Date for submission of entries is 01.10.2009 to be sent to head office or mail at
* Entry Fees Rs. 400.00 per Event & Rs. 200.00 for next event (Non Refundable) will be charged.
* Lodging & Boarding Rs. 1100.00 (Inclusive of Hotel Stay from 3rd to 6th October and 3 times meal for 4th and 5th October) will be charged.
* Lodging & Boarding will be provided subject to the confirmation on or before 01-10-2009.
* One Gold, One Silver & Two Bronze Medals along with Merit Certificate will be awarded to Winners.
* An Attractive Memento will be presented to all Official Instructors.
* All participants should be in clean Karate uniform with their own protective guards and Aka / Ao Belt.
* Championship will be conducted as Per the New Rules of WKF amended from 01.01.2009.
* Below 15 Yrs Age categories events will be conducted on 4th October and 16 Yrs & above will be on 5th Oct.
* Championship will start sharp at 9.00 AM both the days and concluded at 8.00 PM respectively.
* Please note that Organizers has the right to alter/cancel any event in case of unavoidable circumstances.
* Organizers has the right to refuse the entry of any Individual/Team without explaining any reasons.
* Organizer/Any Individual will not be responsible in case of any injury during the Championship.

CONSENT/INDEMNITY FORM FOR PARTICIPATION
I______am aware that karate is a contact sport and that injury may occur in the course of participation. I am also aware that the term injuries include injuries of every description including temporary disablement, permanent disablement as also loss of life.
I authorize and concerned to being rendered all medical treatment in case of any injury/accident during travel/competition/practice by Shito Ryu Seiko Kai Karate Do India and those associated with it. I agree to reimburse the cost of such medical treatment and any other incidental expenses so incurred.
