INTRODUCTION

The attached equal opportunities monitoring form is necessary in order to provide statistical information that can help assess the effectiveness of REACH Community Health Project’s equal opportunities policy. The information that you supply on your monitoring form will also be treated in strict confidence and will not be made available to the personnel responsible for the interview and selection procedures. J: JOB SHARING
Job sharing is the voluntary sharing by two persons of all the duties of an established post with the salary and appropriate conditions of service shared on a pro-rata basis according to the hours worked by each job sharer. In certain cases job sharing is also available to current employees who wish to share two related job-share posts in the same department. Where you wish to share both your present post and the post applied for on a job-sharing basis then you must discuss this with your immediate supervisor before submitting an application.

K: HEALTH
All new entrants to the service of REACH Community Health Project may be required to complete a full medical questionnaire, as will all candidates being interviewed for specific posts.
REACH Community Health Project may also require certain new entrants to be medically examined, depending on the duties of the post and/or the circumstances of the person to be appointed. This will normally be discussed during the selection interview stage.
All staff are expected to maintain an acceptable level of attendance. Each department has a target attendance level - details of which will be discussed on appointment.
L: DISABILITY

P: EXPERIENCE, SKILLS AND TRAINING
The importance of this section to the selection process is emphasised by the space allocated. Using typescript or handwriting, this is where you have the opportunity to state your case - explain in your own words why you think that you, more than anyone else should be offered the post.
