APPLICATION FOR PERSONAL IMPORT/EXPORT LICENCE

Notes:

Pages 2 and 3 of this application will be processed to become the licence document which must be carried by the person travelling.

  1. Please delete “Importation” or “Exportation” as appropriate.
  2. Please complete all fields. A list of commonly encountered controlled drugs[1] can be found on the Drugs Licensing website.
  3. The form must be accompanied by a brief note from the appropriate doctor/addiction service, on headed notepaper, confirming that the details given are correct.
  4. Completed form(s) should be e-mailed to the LicensingTeam in the original Word format(contact details above).
  5. All requests for personal licence(s) should be sent at least 10 working days in advance of travel.
  6. Please note that PDF licence(s)may not be accepted by UK Border Force or overseas authorities.
  7. “Drug name” should be the controlled drug contained in the preparation. The brand name may also be listed.
  8. “Form” should refer to tablets, capsules, ampoules/mixture etc.
  9. “Strength” should refer to the drug content in each dosage unit or weight/volume ratio in a liquid.
  10. “Quantity” should refer to the total amount of tablets, capsules, etc.

Licence for Personal *Exportation/Importation of Controlled Drugs

In pursuance of section 3(2)(b) of the Misuse of Drugs Act 1971, the Secretary of State hereby authorises the person named below to export from or importinto the United Kingdom the controlled drug(s) preparation(s) shown below in the amounts specified and;to import into or export from the United Kingdom any residue of the drug(s):

Conditions

  1. It is an offence under Section 18(2) of the Misuse of Drugs Act 1971 to contravene a condition or other term of this licence.
  2. It is an offence under Section 18(4) of the Misuse of Drugs Act 1971 for the purpose of obtaining this licence for himself or another to give false information.
  3. This licence may be modified or revoked by the Secretary of State at any time.
  1. The drug(s) may be exported/imported by the licensee solely for their own use as prescribed.
  1. This licence shall be produced on demand of a Constable or HM Customs Officer.
  1. If this licence is modified or revoked it shall be surrendered within 7 days to Home Office Drugs Licensing at the address above.
  2. This licence shall be surrendered to Home Office Drugs Licensing within 7 days upon return to the UK.
  1. By virtue of Regulation 22(4) of The Misuse of Drugs Regulations 2001 no record need be made by the licensee of any quantity of a controlled drug exported or re-imported by him/her under this licence.

N.B.: This licence is not valid unless it is signed and embossed by Home Office, Drugs & Firearms Licensing Unit.
