Application for an Appeal against a Decision of an ExaminationBoard

This form is to be used by a student who wishes to appeal to the University against the decision of an Examination Board.

This form should be completed and sent onlyto the Academic Registry (Ref: Appeals), Cardiff Metropolitan University, Llandaff Campus, Western Avenue, Cardiff CF5 2YB, or by email to , to arrive within 14 days of the date of thenotification of results by the School. Completed forms MUST NOT be left at the I-Zone.

Students should read the Appeals Procedure carefully, taking particular notice of the eligible grounds on which an appeal may be made. Appeals may not be made on grounds that a student is just dissatisfied or disappointed with an examination result.

The grounds of appeal must be made clearly and concisely. Further details, if any, and any relevant documentary evidence, medical, or otherwise, must be attached.

Give the date of the Examination Board, against the decision of which you are appealing:
State the Examination Board’s decision against which you are appealing:
State concisely the grounds on which you wish to appeal (i.e. exceptional personal circumstances or alleged defects/irregularities in the conduct of the examinations, or both).

If applicable, describe the exceptional personal circumstances which affected your performance in the assessment(s) / examination(s) and which were not known to the Examination Board when it reached its decision because you had not reported them before the Board via the Mitigating Circumstances Procedure.
Identify modules affected using module names/numbers and give precise dates and details of any events (such as illnesses or bereavements). Medical or other documentary evidence must be attached. (Please continue on a separate sheet if required).
State the reasons why you could nothavemade your exceptional personal circumstances known to the Examination Board by submitting a Mitigating Circumstances Form before the meeting of the Board.
If applicable, describe anyalleged defects or irregularities in the conduct of the examinations or other assessments or in written instructions or advice relating thereto, showing how these had an adverse effect on your performance.
Identify modules affected using module names/numbers and give precise dates and details of any events (such as meetings with supervisors, submission dates). Documentary evidence to support any allegations must be attached. (Please continue on a separate sheet if required).
