**Note: To apply for trips outside the U.S. you MUST attacha copy of your passport to your application.The passport must be valid six months beyondthe travel dates. Applications without passport copy attached will not be considered.
**Also, if you are abroad in the fall 2011 semester, you will not be able to apply for the January trip. If you are abroad in the spring 2012 semester, you will be unable to apply for the spring and summer trips.

I. Please write an essay responding to each of the following; limit yourself to one page per question.
A.Reflect on how service to others has been a part of your life, especially over the last three years. Be specific about how you have given of your time and talents and reflect as well upon what you have learned from this experience.
B.Explain the primary reason(s) that you wish to go on a Miller Center Mission Trip this year. What do you hope most to gain from this experience? What do you hope to offer?
II. Please provide two letters of recommendation: one from a non-Hendrix associate who is familiar with your character and goals (cannot be a family member), and one from a Hendrix faculty or staff (freshmen may substitute a letter from a high school teacher or counselor). It is YOUR RESPONSIBILITY to see that your recommenders send us their letters by the deadline.
III. Please complete and attach the Confidential Information Waiver!
IV. Attach proof of valid passport if applying for trips abroad.

I, ______, request that the Office of Student Development and the chair of the Committee on Academic Integrity provide, both to the Chaplain and to the Miller Center Staff, access to any relevant information in my personal file. This permission is given with the understanding that all such information is confidential, is to be used only in order to make determinations of importance to the placing and support of the student participating in trips sponsored by the Office of the Chaplain and the Miller Center, and that the information is to be requested only when necessary.
