Appendix 2: Bidder’s Qualification Statement

Each bidder must submit a response to these minimum requirements to allow the selection committee to evaluate the bidders. If submitting a proposal as an individual company, whether the proposal is for a complete project or for a subsystem or component; this form should be filled out in its entirety. If a proposal is being submitted by multiple companies working together as a team (includes prime, subcontractors, suppliers, etc. regardless of relationship); the form should be filled out once with a response based on the qualifications of the team and then additional forms shall be completed and submitted by each member of the team. Thus, if a team of five companies are submitting a combined bid package, there would be six Bidder’s Qualification Statements submitted. A response to a single question should not take more than ½ Page unless there are extraordinary circumstances. Please use a blue font for your responses.

  1. Prior GPA Projects. List all projects completed on GPAterminals within the last five years. Do not include maintenance, repairs or replacements of previously installed items unless the total value was greater than $100,000. Include the dollar amounts and the contact information of the prime contractor.
  2. Port Projects. List all projects completed in any port terminal within the last five years. Do not include maintenance, repairs or replacements of previously installed items unless the total value was greater than $100,000. Include the dollar amounts and the contact information of the prime contractor and the contracting officer for the terminal. Projects at GPA terminals can be counted for this section too (you may reference question 1).
  3. Previous Installations. List all projects completed where the systems / equipment installed are the exact components / systems to be installed with this project. Include the dollar amounts and the contact information of the prime contractor and the contracting officer at the location identified. Also identify whether or not the installations are still in operation. If they are no longer in operation, please explain why not.
  4. Company Age and Financial Condition. Describe your company history including dates, mergers, acquisitions, and other means that have been used to grow your company over the years. Include all names under which the company has operated. Also include a statement regarding your financial condition. Prime contractors shall make a statement regarding their ability to withstand a default by your subcontractors.
  5. Teams. Where members of the team have worked together on previous projects, describe those projects. Include the dollar amounts and the contact information of the prime contractor and the contracting officer at the location identified. If multiple projects have been completed by the team, only list the five that are the most similar to this project.
