NEW STATE LAW REQUIRES PARENTS TO SIGN FORM REGARDING CONCUSSION AWARENESS

Michigan recently became the 39th state to enact a law requiring all school districts and sports-related organizations to provide educational materials about concussion awareness and the risks of concussions to each player. The law also requires parents or guardians to sign a form acknowledging that they have received Concussion Awareness information from AYBA. This signed form must be kept on file with Almont Youth Baseball Association thru out the 2015 playing season.

Waiver of Liability and Medical Release Form

______has my permission to participate in the Almont Youth Baseball Association (AYBA) program. I, the undersigned parent/guardian, assume all risks and hazards incidental to participating in this activity and do hereby waive, release and hold harmless the Executive Board of AYBA, their managers, assistant coaches, sponsors and participants from any claim arising out of injury to my child. In the event of an accident or illness to our child, we authorize the identified representative of AYBA to obtain such medical diagnostic services as may be deemed necessary. Emergency treatment of a life threatening condition is authorized. Telephone contact for management of all serious conditions will be attempted if possible.

Parent Fundraising and Volunteer Agreement

I, a parent/guardian of the player that I am registering for AYBA, understand that said organization is run on a complete volunteer basis and that every parent/guardian has the required duty to volunteer for at least one needed position (average of 2 hours) during the recreational season. I/We are also aware that fundraising is vital to AYBA to provide essential equipment, maintenance and other necessary costs indispensable to the operation of a youth baseball league. My/Our child will meet the fundraising goal of FOUR (4) kits per registered family sold thru Little Caesar’s, OR I/We agree to pay the difference of $20 per registered family if this goal is NOT met. By my signature below I hereby acknowledge my commitment to volunteer at least once during the season and also will abide by the above stated fundraising requirement. I recognize that my failure to comply with the fundraising commitment will cause my child’s uniform to be withheld until such time that I then pay the required $20.

PARENT CODE OF CONDUCT

We, the Almont Youth Baseball Association Board of Directors, have implemented the following Parent Code of Conduct for the important message that it holds about the proper role of parents in supporting the youth of our community and their child in sports.

The essential elements of character building and ethics in sports are the foundations of sportsmanship and are demonstrated by the core principles of respect, responsibility, fairness, and good citizenship.

As an AYBA Parent or Guardian, I hereby pledge to provide positive support, care, and encouragement for each child participating in AYBA by adhering to the following code of conduct:

  • I will encourage good sportsmanship by demonstrating positive support at all times.
  • I will remember that children participate to have fun, and that the game is for youth, not adults.
  • I will not engage in any kind of unsportsmanlike conduct with any umpire, Board member, coach, player or parent such as booing and taunting, refusing to shake hands, or using profane language.
  • I will teach my child that doing one's best is more important than winning.
  • I will respect the officials and their authority during games.
  • I will not confront coaches or managers at any time during games.
  • I will refrain from coaching my child or other players during games.
  • Alcoholic beverages and smoking are prohibited on school property.

I also agree that if I fail to abide by the aforementioned rules and guidelines, I will be subject to disciplinary action levied by the AYBA Board of Directors or Code of Conduct Committee that could include, but is not limited to the following:

  • Verbal Warning
  • Parental suspensionfor games
  • Parental season suspension
  • Parental removal from all league activities

The Almont Youth Baseball Association Board of Directors retains the sole authority to determine what, if any, disciplinary action is appropriate for any conduct infraction.
