Instructions MCASCP Badging

All subcontractors requiring access to MCAS Cherry Point:

1. Email a list of employees requiring badges to (first, middle, last name only). I will e-mail a sponsor letter and base table to Cherry Point Pass & ID.

2. Complete the required forms (authorization for release of information, new background form effective 6/10/14**, and ASO 5512 PMO).On the ASO 5512 PMO form, your company name should be inserted where it ask forCompany/Sponsor’s Name. Subcontractors no longer have to pull and submit background checks on their employees for MCAS Cherry Point vetting as of 6/10/14. Please do not send the background check to Cherry Point. Please do not confuse this with Camp Lejeune’s vetting policy which has not changed.

3. Each subcontractor is responsible for mailing (FEDEX, UPS or EXPRESS Mail)or hand deliveringthe three forms (there are four pages) listed above in #2:

4. Please allow sufficient time for delivery by mail and five (5) days processingby the Provost Office.

5. When they arrive at Cherry Point for badging each employee must have the following information:

-Picture ID (typically a driver's license)

-Social Security Card or official document listing SSN

-Birth Certificate, Passport, DD FM 214 or INS card to verify citizenship

-Provide proof of immigration status (If NOT US citizen)

-Vehicle Registration (if they are requesting a vehicle decal)

-Vehicle Insurance Proof (if they are requesting a vehicle decal)

-Sponsor Letter (if it wasn’t emailed to ,and most of them are)

Also when your employees arrive at the ID Center they will need to give the attendant the job site name for which they are being badged and your company name for them to look their information up in their system.

** Please understand that while Cherry Point no longer requires you to submit your background check to them, this form (new background form effective 6/10/14) must be complete and correct. Probably best to do a background check to complete this form with if you or your employee have any doubts whatsoever that the information you have provided may not be 100% correct, any exclusions or errors are an automatic denial.
