Student Classroom Information Sheet for TCCs and Instructors
The following page is an information sheet we ask that you distribute to every student who takes a classroom PALS or PEARS course at your center through this September.
Before you distribute the information sheets to the classroom students, please:
- Circle the course that they completed.
- Complete the four lines that begin with “Name of training center.”
- Copy and distribute to each student who takes classroom training between
April 15and September 30, 2013.

Thank you for taking an American Heart Association emergency cardiovascular careclassroom course at our Training Center.
To help us improve the effectiveness of the course, we are asking PALS and PEARS students to take a very quick online survey about the training.
Within the next 5 days, please go to this URL so that you can access the surveyabout your course.
When you are completing the survey, you will be asked to provide the following:
Name of the course you took:
- PALS, or
- PEARS
Name of Training Center: ______
The Training Center’s ID number: ______
The name of your Instructor: ______
Date of your training: ______
To thank you for completing the survey, you will be entered into a monthly drawing to receive a $150 VISA Gift Card.
