CARDHOLDER AGREEMENT
I, acknowledge receipt of an IIT/ PNCBank VisaCard Corporate Procurement Card, number xxxxxxxx0000-0000 with a monthly credit limit of$0000.
As a Corporate Cardholder, I agree to comply with the terms and conditions of this Agreement and the “Cardholder’s Guide” the current copy of which can be as found on the IIT Purchasing Departmentwebsite at:
I acknowledge receipt of a copy of the Guide and confirm that I have read and understand the terms and conditions therein. I understand that IIT is liable to PNC BANK VisaCard for all IIT charges.
I agree to use this card for IIT-approved purchases only and agree not to charge personal purchases. I understand that IIT willaudit the use of this card and report any discrepancies.
I further understand that improper use of this card may result in disciplinary action, up to and including termination of employment. If I fail to use this card properly, I understand that I will be personally liable for all improper charges, and I agree that I will make arrangements to pay the same, either in a lump sum or by executing a payroll deduction agreement authorizing IIT to deduct from my salary an amount equal to the total thereof. I also agree that I willremain personally liable for any such improper charges even if I am no longer employed by IIT.I understand that my failure to account to IIT for any improper charges may not only result in disciplinary action, up to and including termination of employment, but also the initiation of legal action to recover the amount of such charges. I further agree that if IIT initiates legal action to recover any such amountsand I am adjudged liable for then I shallbe liable forthe expenses that IIT incurs in connection with such recovery, including, without limitation,costs and reasonable attorney fees.
I agree to register for access to PNC Visa IntelliLink Spend Managementon line ( , be trained in its use, and review the transactions recorded to my account on at least a monthly basis.
I understand that IIT may terminate my rights to use this card at any time for any reason. I agree to return the card to IIT immediately upon request or upon termination of my employment.
