This book was designed to assist you in identifying drugs and their uses, furnish you with various medical and insurance related definitions, and offer suggestions for special medical questions related to certain impairments. These will help you select the best applicants and you canmove on to yournext client.

The applicant and you are the best sources of information available to assist us in the compilation of a complete and accurate personal health profile. As a Company, we value your partnership and will endeavor to meet your needs by providing relevant, timely, and updated information.

SPECIAL MEDICAL QUESTIONS

Review the heading. If your applicant indicates any of the following situations, here are questions to help you proceed:

CHECKUP

  1. What was the date and reason for the checkup?
  2. What symptoms prompted the checkup?
  3. What tests were done and what were the results of each?
  4. What medication was given?
  5. Were any further studies recommended or planned?

ARTHRITIS

  1. What type of arthritis such as rheumatoid, osteo, gouty, juvenile, etc?
  2. What joints are involved?
  3. Is there any swelling or joint deformity?
  4. What medication or treatment is given?
  5. What was the date of the most recent arthritic episode?
  6. Are there any restricted activities?
  7. Is it disabling?

RESPIRATORY CONDITIONS INCLUDING : ALLERGIES, ASTHMA, EMPHYSEMA, BRONCHITIS

  1. How many attacks occur per year?
  1. What was the date of the last attack?
  2. How disabling are the attacks?
  3. Are any days lost from work or school and if so, how many?
  4. What medication is used to control the attacks?
  5. Between attacks, what medication is used and how often?
  6. Are any injectable drugs required and if so, what?
  7. Has hospitalization ever been required and if so, when?

9Is tobacco used and if so, what kind?

BACK AND NECK

  1. What areas of the back and neck are affected?
  2. Was the applicant unable to work and if so, for how long?
  3. How long did the symptoms last?
  4. How is the ailment treated and is there any current treatment?
  5. What medications were prescribed and are they still used?
  6. When was the doctor last seen?
  7. Is the applicant discharged from the physician’s care?
  8. What was the date of the last symptom and what was the symptom?
  9. Was there any radiation of pain or numbness down the arms or legs?
  10. Was the condition disabling or was disability benefits received or applied for?

BLOOD PRESSUSRE/HYPERTENSION

  1. How long has the applicant been under treatment?
  2. How often is the blood pressure checked and by whom?
  3. What medication is taken (name them) and how often?
  4. What are the latest readings?
  5. When was the condition diagnosed?

EIPLEPSY, SEIZURES OR FAINTING SPELLS

  1. Describe the type of seizure, epilepsy or fainting spell.
  2. What were the dates of the first episode and last episode?
  3. How often do symptoms occur?
  4. What studies have been done, and what were the dates and results of the studies?
  5. How often is follow up required?
  6. Name the medications taken and when was the physician last seen for this condition?

HEART MURMUR

  1. When was the condition diagnosed?
  2. What type of murmur is it such as systolic or diastolic?
  3. How was the murmur diagnosed and describe the tests used and the date.
  4. What doctor ordered the tests and what doctor has the applicant seen most recently?
  5. Are activities restricted and if so describe?
  6. What medication is required?

URINARY TRACT DISORDERS INCLUDING KIDNEY AND BLADDER

  1. What is the name of the disorder or disorders?
  2. Have many episodes have occurred and what dates?
  3. What tests have been done and what were the results?
  4. Is any surgery or continuing treatment recommended?
  5. How was the condition treated and what medications were used?
  6. Are medications still used and if so, what?
  7. If kidney stone was present, was it passed, surgically removed or still present?
  8. If kidney stone was surgically removed, how was this done such as open surgery or lithotripsy?

MENTAL AND EMOTIONAL DISORDERS

  1. What was the diagnosis?
  2. How was the condition treated and by whom?
  3. Was hospitalization required and if so, for how long?
  4. Were counseling or psychotherapy used and if so explain type of treatment and dates.
  5. What medications were used and what is currently used?
  6. If medication was discontinued, what was the date last used?
  7. Was there any disability and if so, when and for how long?
  8. Is the applicant still under medical care and if so, with whom?
  9. If applicant has been discharged from care, what was the date?

STOMACH, INTESTINE, COLON DISORDERS

  1. What was the diagnosis and if an ulcer, what type such as peptic, esophageal, stomach or duodenal?
  2. How is the condition treated and how long has it required treatment?
  3. When was medication last taken and what was the medication?
  4. Is a special diet required and if so, what type?
  5. Has there been bleeding from the gastrointestinal tract and if so, when?
  6. Has hospitalization been required and if so, when and for how long?
  7. Was surgery needed and if so, describe what was done.
  8. Was gastritis, irritable bowel, hiatal hernia, or esophageal reflux or stricture present and if so, which and how was it treated?
  9. Is there a history of ulcerative colitis, Crohn’s Disease, ileitis or inflammatory bowel disease?

THYROID DISORDERS

  1. Is the problem hyperthyroid (overactive) or hypothyroid (underactive)?
  2. Are nodules or goiter present?
  3. How is the condition treated? Name the medications or types of surgery.
  4. Are there any current symptoms and if so, what are they?
  5. Is there a history of thyroiditis?

TUMOR, POLYP, CYST

  1. What type of growth was it and is it still present?
  2. Where is the growth located and describe site and organ.
  3. How was the growth removed such as surgery, burned off, topical medication or radiation?
  4. At what facility was the growth removed such as doctor’s office, outpatient clinic or hospital?
  5. What did the doctor call the growth or what was the pathological diagnosis?
  6. Was any treatment or follow up needed after the growth was removed?
  7. Was the growth diagnosed as cancer and if so, what kind?

OTHER CONDITIONS

  1. When was the doctor seen and give all dates?
  2. Why was the doctor seen?
  3. What symptoms or complaints prompted the visit?
  4. What tests were done and what were the results?
  5. What did the doctor call the disorder?
  6. Was medication prescribed and if yes, what was the name of the drugs? (These will be on the medication container).
  7. Is medication still used and if not, when was it last taken?
  8. Are there any remaining symptoms or signs of the disorder and if yes describe them and when they occurred?
  9. Is there any residual impairment or disability?
  10. Were any other doctors seen? (If yes, repeat drilldown of above questions.)

CONSTRUCTION

  1. What kind of employment industry does the applicant work?
  2. Is Worker’s Compensation available?
  3. Is the applicant covered by Worker’s Compensation?
  4. Describe the job duties.
  5. Describe the daily activities.
  6. Where is the job site or location?
  7. Describe the equipment used and amount of time using it on the job.
  8. Have there been any on-the-job injuries in the past 2 years and if so, describe them?
  9. How long has the applicant been employed in this position?
  10. Are any certificates or licenses required?

TRUCK DRIVING

  1. What is the average number of miles driven per day?
  2. Do you drive outside your state and if yes, where and how often?
  3. Have there been any traffic violations in the past 3 years and if yes, what were they and give dates.
  4. Are there any overnight stays?
  5. What type of vehicle is used on a daily basis?

GENERAL

  1. What is the activity or avocation?
  2. How often is the activity or avocation performed?
  3. Are any special licenses or certificates needed?
