Job Title: 50 Plus Engagement Worker

Reporting to: Commercial Services Manager

Specific Responsibilities and Duties:

To support people aged 50+ in Redcar & Cleveland to make positive health & lifestyle choices and to sustain appropriate behavioral with a focus on

·  reducing alcohol consumption

·  smoking cessation

·  eating healthily

·  increasing physical activity

To find ways of enabling clients to overcome the barriers preventing them from engaging more fully in their community.

To be responsible for the delivery of the project.enabling 480 people aged 50+ to engage in community based activities, utilizing the Warwick Edinburgh screening tool with an incorporated Audit C to identify a minimum of 20 people aged 50+ living with or at risk of developing alcohol or substance issues

Working with people aged 50+ develop 6 peer supported interest based community activities, particularly working with men

Together with ‘Lifeline’ raise awareness of alcohol and substance misuse with 1000 residents of Redcar & Cleveland

To liaise effectively with health and social care professionals to raise awareness of Age UK Teesside’s existing services, activities and development opportunities as a recognised element of the health and social care preventative programme and to identify future partnership and funding opportunities.

To be the key contact person for health colleagues to engage with and refer clients to, set up initial appointments, identify realistic goals and develop actions plans to ensure agreed health outcomes are achieved.

To refer individuals to other Age UK Teesside services or partner agencies to achieve health lifestyle outcomes.

To monitor and evaluate both the client’s outcomes and the relationship with the health professionals, Lifeline and partner agencies.

Responsible for reporting any safeguarding concerns regarding individuals.

To attend and participate in internal/external meetings, as appropriate.

To work closely with other members of the staff team, keeping abreast of developments, sharing contacts, communicating plans and ensuring that Health & Wellbeing activities are fully integrated within the organisations.

Under the Health & Safety Act at Work Act 1974 and associated guidance, it is the duty of all staff while at work to take adequate care for the health and safety of themselves and of other persons who may be affected by their acts or omissions.

To work with other staff to ensure appropriate customer care is provided at all times.

To develop a personal knowledge base on Age UK Teesside to underpin effective working.

The post may involve occasional evening and weekend work for which time in lieu can be claimed.

Strong communication skills – verbal and

NVQ 3 in Health & Social Care / Community Development
