GUIDELINES FOR SYSTEM EVALUATION:

1. EFFECTIVE: Setup/equipage correct with few deficiencies. Publications are updated.

2. PARTIALLY EFFECTIVE: Setup/equipage has some deficiencies but achieves the basic goals. Specific deficiencies have been identified.

3.NOT EFFECTIVE: Program is not maintained and significant discrepancies noted. Serious deficiencies which prevent the program from serving its intended purpose; or has an accumulation of minor deficiencies which together severely impair effectiveness; or there is no functional program in practice. An immediate, intensive training effort is required for all personnel involved to enable the program to regain effectiveness. Close oversight by senior personnel/ICIS is required
