About Medicare Part D: AMOS has provided information for you to use in making your own decision. When choosing, consider: (a) Medicare’s estimate of total annual cost, (b) premiums, (c) annual deductible, if any, (d) drugs that will and won’t be covered, (e) co-pays, and (f) insurance coverage, if any, during the “doughnut hole.” If you choose a Medicare Advantage type of plan, also check to see if your doctor(s) and hospital are in the plan’s network. Many hospitals and doctors accept only a few of the Medicare Advantage plans.

An AMOS volunteer will call you in 2–3 months to see how you are doing. In the meantime, if you have any questions or need help with application forms, call us at (865) 524-2786.

Talk to your doctor about free samples, generics, lower-priced name brands, and possible over-medication.

Enroll in a stand-alone Medicare Part D Prescription Drug Plan for prescription drug coverage. Use the information AMOS has obtained from Medicare to choose the plan that’s best for you.

Enroll in a Medicare Advantage plan that includes Part D prescription drug coverage. Use the information from Medicare to choose the plan that’s best for you. Make sure your doctor(s) and hospital are in the plan’s network. Medicare Advantage plans are a “package” of coverage.

As you requested, AMOS applied for Extra Help (Low Income Subsidy) for you for Medicare Part D.

Apply to Patient Assistance Programs (PAP) from drug manufacturers to get their free brand name medicines for people with (a) limited income, (b) no government assistance, (c) and no prescription drug insurance coverage. For help completing the enclosed applications for these programs, please call AMOS.

Apply to Tennessee Department of Human Services for help with Medicare Part B expenses through QMB, SLMB, or QI.

Apply for the VA health care program including prescription drug benefits for veterans.

Use discounts or discount cards.
