AFCENT Waiver Guidance

Guidance/ forms for AFCENT SG waiver approval into CENTCOM AOR

o  Waivers are valid for 12 months upon date of entering AOR.

o  Waivers cannot expire while in AOR

o  AFCENT deployment waiver NAMING FILE (LASTNAME FIRSTNAME LASTFOUR AFCENT WAIVER)

o  Orthodontics form NAMING FILE (LASTNAME FIRSTNAME LASTFOUR ORTHODONTICS)

o  Other medical supporting documents NAMING FILE (LASTNAME FIRSTNAME LASTFOUR SUPPORT DOCS)

o  CPAP letter of understanding NAMING FILE (LASTNAME FIRSTNAME LASTFOUR CPAP LOU)

Please follow naming convention as we receive 130+ waivers a month for processing

o  DO NOT manipulate the PDF waiver form (separate it into parts – it will not import)

o  The checklist is MANDATORY to be completed for all waivers. Please fill it out COMPLETELY

o  Only fill out the portion of the waiver that is applicable to the medical condition

o  Sign and date the waiver package

Forms must be filled out electronically NOT hand written and scanned ( Will NOT import into MEDXS waiver system)Scanned forms will be sent back to be completed correctly

o  Forms must be COMPLETE and ACCURATE

Incomplete forms will be sent back to be corrected

Prior to waiver submission member will require:

Active Duty:

o  PCM clearance note in AHLTA stating that member is medically stable to deploy with waiver approval

o  MH clearance if applicable with note in AHLTA stating member is stable to deploy with waiver approval

o  If seeing a civilian provider off base please provide latest note in PDF

o  HIV 120 days prior to RDD. Must be good through deployment ( it is an annual requirement)

o  Specialty clearance note if applicable (ortho, GI, sleep clinic etc) PDF and send in separate document if seen by off base provider and documents are not in AHLTA

o  RILO good through deployment if applicable – if expires near end of deployment may just have an extension granted versus going through an entire RILO process 6 months early.

o  422/469 if applicable

o  Full set of vital signs and date within past 3 months

o  OSA waivers are dependent on several factors: multiple comorbidities (HTN, hyperlipidemia, obesity)/deployed location. Some locations cannot support CPAP equipment due to unreliable power source.

ANG/Reserves:

o  ANG/Reserve unit narrative summary and recommendation

o  PDFs from civilian PCM and specialist

o  PDFs from civilian MH providers.

o  HIV 120 days prior to RDD. Must be good through deployment (it is an annual requirement). Date of HIV drawn is fine – just let us know.

o  Specialty clearance note if applicable (ortho, GI, sleep clinic etc) PDF and send in separate document if seen by off base provider and documents are not in AHLTA

o  RILO good through deployment if applicable – if expires near end of deployment may just have an extension granted versus going through an entire RILO process 6 months early.

422/469 if applicable

o  Full set of vital signs and date within past 3 months

o  OSA waivers are dependent on several factors: multiple comorbidities (HTN, hyperlipidemia, obesity)/deployed location. Some locations cannot support CPAP equipment due to unreliable power source.

Civilians/Contractors:

o  GS civilians go through base public health to determine deployment criteria and waiver needed

o  Tracked through AFPC/CEW (Lt Col Romero Reid)

o  They have a choice of going to their PCM or seeing a base PCM

o  Will required medical documentation from civilian providers as well in PDF format

o  Full set of vital signs within the past 3 months

o  OF 178 completed
