PIR – Support Facilitator – Narrogin, Part Time

The Opportunity:

This position will be part of the Partners in Recovery program, which is dedicated to supporting those with severe and persistent mental illness develop and maintain relationships within the community to sustain optimal health and wellbeing. You will be required to promote and enable confident access to services available, ensure all work that is undertaken is aligned and compliant to all legislative and contractual requirements, contribute to a professional and effective team, advocate on behalf of the consumer, and provideindividualisedandflexible support through the health care system. Initially this position will be for a 6 month contract, 20 hours per week.

The Skill Set:

-Previous experience within Mental Health or relevant human services field is essential, coupled with a demonstrated understanding of the provision of Mental Health Services in WA.

-Demonstrated ability of working with individuals who experience mental distress to promote a high expectation of Recovery and facilitate change.

-Highly developed brokerage skills and strong community networks.

-Hold a positive and contemporary attitude towards people experiencing mental distress.

-Excellent written, verbal and interpersonal skills

-Strong demonstrated ability to operate within organisational guidelines, procedures and policies

POSITION SUMMARY

You will be required to promote and enable confident access to services available, ensure all work that is undertaken is aligned and compliant to all legislative and contractual requirements, contribute to a professional and effective team, advocate on behalf of the consumer, and provide individualised and flexible support through the health care system.

KEY FUNCTIONS AND ACCOUNTABILITY OF THE POSITION
  • Enable persons living in the community who have a severe and persistent mental illness to confidently access the services available to them on their journey of recovery.
  • Establish and develop working relationships with formal and informal community groups and services.
  • Work within policies, procedures and guidelines established by the organisation to achieve the above.
  • Contribute as a part of the team to the constant improvement of the organisation's services.

KEY RESPONSIBILITIES
  1. Promote and enable persons living in the community to confidently access the services available to them - Individual Recovery Support

In consultation with and under supervision of the Host Agency Manager create a recovery enabling environment through the following:

  • Develop and maintain a respectful, positive and Client centered relationship with the individual accessing the service.
  • Provide individualised and flexible support to consumers as they navigate the health care system, advocating for them as they access the services they are eligible to receive.
  • Develop, facilitate and regularly review person-centred recovery plans, tools and measures.
  • Assist the Consumer to overcome barriers to accessing services, linking them with relevant services.
  • Have an excellent knowledge of what services are available in the local region and refer consumers to recovery enabling services and community resources as indicated.
  • Establish and maintain respectful, collaborative and flexible but professional relationships and boundaries with staff members, family members, carers and other relevant external stakeholders.
  • Work with existing case managers (not replacing them)
  • Develop effective partnerships and collaborative working styles with key stakeholders to build referral pathways and networks of services.
  • Work with the Host Agency Manager and PIR Integrity Team to allocate Flexible Funding to support PIR Consumers, in accordance with PIR guidelines.
  • Involve family members, carers and other relevant stakeholders in recovery planning.
  • Respond to clinical incidents in accordance with the organisation’s policies & procedures.
  • Contribute to the development and maintenance of networks and partnerships with community groups and organisations.
  1. Ensure all work undertaken Aligns and Complies with RF Policies & Procedures, Contractual Obligations and all Legislative requirements
  • Adhere to organisational policies and procedures and staff Code of Conduct.
  • Maintain own records and contribute to accurate documentation (e.g. administrative tasks).

SELECTION CRITERIA

Essential

  • Have a sound understanding of the provision of Mental Health Services in WA and of Community Health Services broadly and can assist a consumer to navigate this system whilst still promoting consumer independence.
  • Previous work in a relevant human service field working with disadvantaged members of the community
  • Have a clear understanding of Recovery and hold a positive and contemporary attitude towards people experiencing mental distress.
  • Demonstrated ways of working with individuals who experience mental distress to promote a high expectation of Recovery and facilitate change.
  • Excellent written, verbal and interpersonal skills, including the ability to communicate clearly and respectfully with a community of stakeholders (consumers, family, colleagues, other agency personnel and external stakeholders).
  • Strongly demonstrated ability to plan, organise and manage one’s own workload
  • Demonstrated ability to work flexibly across autonomous and teamwork settings.
  • Strongly demonstrated ability to operate within organisational guidelines, procedures and policies.
  • Demonstrated computer skills, with experience in MS Office applications.

Desirable

  • Awareness of relevant mental health agencies and services (both Government and Non-Government).
  • Previous work with community members in an outreach setting (e.g. home visiting).
