Adoption Contract – Puppy
(under six months of age)
The following agreement specifies what we at Bright Eyes Dog Rescue feel is the minimum required to provide a safe and loving home for a dog. The details of the contract reflect our concern that both owner and dog have a happy life together. The requirements are based on years of experience and a continuing commitment to see that unfortunate dogs without homes find safe, happy and permanent homes. We hope that you will understand that considerable emotional involvement, time and energy go into insuring that our rescue dogs have the best possible second chance at life.
“Terms of Adoption”
In the adoption of ______, I agree to the following:
1. To keep this dog in my personal possession and to provide sufficient food, water, shelter, grooming and humane treatment at all times. To keep this dog as my household companion and friend.
2. I agree to have this dog spayed or neutered at my expense within thirty days of the date upon which the dog turns six months old. Proof of spaying or neutering will be provided in writing containing a licensed veterinarian’s signature to Bright Eyes Dog Rescue.
3. I will not allow this dog to breed or to be bred under any circumstances.
4. To provide veterinary care to ensure the ideal health of the dog. To keep current on vaccinations. To procure veterinary care if the dog becomes sick or is injured.
5. That I will provide adequate living conditions for the dog and that the premises where the dog is kept may be visited by one of the Bright Eyes Dog Rescue coordinators at
any time.
6. That this dog will never be chained outside for any reason, or left outside unattended in a yard without secure fencing and adequate shelter and water. I understand that this dog will not live as an “outside dog.”
7. I will ensure the safety of this dog by always having it on leash when in an unfenced area, and/or ensure adequate training so the dog will not get lost or have the opportunity to run away when in off leash areas. Under no circumstances should this dog be allowed to wander or roam free.
8. To provide this dog with an ID tag secured to a collar to be worn by the dog. Bright Eyes Dog Rescue recommends tattooing or micro-chipping as a means of permanent identification. To obey animal control regulations governing the area in which I live.
9. That I will never allow this dog to be transported in the open bed of a pickup truck or other similar vehicle.
10. If at any time I can no longer care for this dog, am unhappy with this dog, or am unable to keep this dog I agree to return the dog to Bright Eyes Dog Rescue (at my own expense). I understand that the adoption fee paid for this dog is non-refundable.
11. I will not abandon, dispose of, sell, trade, or give this dog away to anyone else.
12. In the event of transfer of ownership due to family of personal crisis I will notify Bright Eyes Dog Rescue
13. If for any reason other than severe illness there is cause for this dog to be destroyed, I will notify Bright Eyes Dog Rescue prior to euthanasia.
14. If I move I will notify Bright Eyes Dog Rescue with my new address and phone number.
15. That this dog will not be given for research, scientific experimentation, or other studies.
16. I agree to never leave this dog unattended in presence of children.
17. I agree that Bright Eyes Dog Rescue and any representatives and anyone associated through an adoption will not be held responsible for any destructive or aggressive behavior exhibited by the dog or any actions of the dog, me or my family, pursuant to the adoption.
18. I agree to assume full responsibilities for this dog’s actions and that adoption of this dog is a lifetime commitment. I understand that if I do not follow the terms of this contract I will be in violation of the contract and may be subject to confiscation of the dog by Bright Eyes Dog Rescue coordinators.
19. I understand that if the dog is found abused or neglected I will be liable for prosecution to the full extent of the law. Any legal action concerning this dog, Bright Eyes Dog Rescue and its representatives must be filed and held in Regina, Saskatchewan, Canada.
20. I agree to pay the adoption fee of $350 ($250 adoption fee and $100 spay/neuter deposit) set forth by Bright Eyes Dog Rescue for this dog. The spay/neuter deposit will be returned to the adopter once proof of sterilization has been be received by a Bright Eyes Dog Rescue representative. This fee is refundable within 2 weeks of signing of the contract. The 2 weeks are considered the trial period, after which the above adoption fee is not refundable.
21. Bright Eyes Dog Rescue requires obedience classes for any adopted puppy.
