Student Level(DUE 5/19)

REVIEW POLICIES WITH STAFF

“District must provide evidence…” These are items that should have happened before the meeting, so they can’t be fixed in the present

Examples:

  1. Provide parents with notifications of the IEP meeting
  2. Provide parents with an assessment plan

Directions: Hold meetings with staff and review all of the policies that were found out of compliance at either the Student or District level. You can use the SELPA developed PowerPoint and/or go to the Item Table and use the Compliance Standards and Guidelines to review procedures with staff (Item Table and PowerPoint found on SELPA SESR webpage under Activity 3). Have a sign in sheet indicating the topics covered, and have all staff sign in. Keep this record in case you are subsequently chosen for a follow up review. Enter the date of the meeting into the software as your date of correction.

ADDENDUMS:

“District must provide evidence that the IEP…” These are items that can be fixed in the present

Examples:

  1. Includes an explanation of the extent to which a student will not participate in general education…
  2. Statement of how progress will measured and reported
  3. Transportation is indicated
  4. Frequency and duration of services are specified

Directions: Send home addendums, file signed copies in cum files, and indicate the date of the addendum in the SESR software.

HOLD IEPS:

“The district must reconvene the IEP team to….”

Examples:

  1. Have at least one general education teacher participate
  2. Indicate modifications and accommodations

Directions: Hold the IEP meeting, file updated IEP in cum file, and indicate the date of the IEP meeting in the SESR software

District Level (DUE 6/20)

  1. Print out the “Summary of # of Students in Item List” to see all areas you must check
  2. While directions indicate to review half of IEPs developed in the last 6 months, Shirley Geddes said to use the 2/28/14 date as your starting point. If you have no IEPs developed since that time, go back no further than 6 months.
  3. Use Worksheet 4*to review files for the items that have been found out of compliance. You can copy and paste the Item # and Compliance Test from the Item table. Put all of the items numbers you will be testing on a master, and then make copies for the number of files you need to pull and put individual student names on the top of each set. You will then indicate whether you found each item to be Compliant, Non-compliant, or Not applicable.
  4. Use Form 4*to just have a complete list of the students’ files you reviewed. Like your original protocols, this form is broken into sections: School age, Identification and Initial Assessment, Low Incidence, English Language Learner, etc… ONLY put their names in the sections that have Item Numbers you need to review
  5. NOTE: Form 4 and Worksheet 4 are not mandatory forms, but maintaining the information that is captured on those forms is. If you would like to develop an alternate format that captures all of the information in case your district is chosen for a follow up review, you may do so.

  • Enter Correction data into SESR software (see CDE PowerPoint under Activity 3 on the SELPA SESR webpage for specific directions)

AFTER YOU RECEIVE AN EMAIL THAT YOUR CORRECTIVE ACTIONS UPLOAD HAS BEEN ACCEPTED

  • Submit a hard copy of the Self Review Assurance Form (SESR Form 2)to the California Department of Education (CDE), Special Education Division (SED):
