ACKNOWLEDGMENT OF RISK - RELEASE OF LIABILITY & PARTICIPANT AGREEMENT @ KIDS PARADISE

I, The parent/legal guardian (court appointed) or custodian of the participant(s), agree that the participant(s) and/ or I shall comply with the terms, rules and conditions for participation in any party and/or program at KIDS PARADISE. I understand that the equipment and activities at KIDS PARADISE have inherent risks and may result in serious injury, paralysis or death.

I further understand that the activities and the equipment will be shared with others over whom KIDS PARADISE has no control; I acknowledge that by participation in the parties and programs at Kids Paradise, and/or use the play area and the equipment creates a risk of injury, and I, on behalf of myself and/or the participant(s), knowingly and freely assume all such risk, both known and unknown, even if arising from the negligence of others.

I agree to RELEASE, DEFEND, IDEMNIFY, NOT SUE AND HOLD HARMLESS KIDS PARADISE. I, for myself and/or the participant(s), and or heirs, assigns, administrators, personal representatives, and the next of kin, hereby release and hold harmless Kids Paradise, their affiliates, officers, members, agent, employees, other participants, and sponsoring agencies from and against all claims, injuries, liability\ies, or damages arising out of or related to participation in any and all Kids Paradise, activities, parties, the use of the play area and/or inflatable equipment or resulting from my voluntary attendance/participation at KIDS PARADISE, or the voluntary attendance/ participations of those for whom I have signed below; and I, for myself, and/or as parent, legal (court appointed) guardian or custodian, willingly agree to comply with the stated and customary terms, rules, and conditions for attendance/ participation, I, for myself, and/or as parent, legal (court-appointed) guardian or custodian, have carefully read the above participation agreement, assumption of risk acknowledgment and release of liability, hereby agree to be bound by it for myself and all minor participants listed below, and fully understand its contents. I have the permission from the minor's family or other responsible party to cover minor under this agreement. In acknowledgement of being allowed to participate in any party/program at Kids Paradise, the undersigned, on his or her behalf, and/or on the behalf of the participant(s) identified below, considers and acknowledges and agrees to all the terms and conditions above. In the event that medical attention is needed for myself, or any of the guests/participants listed below, Kids Paradise will call 911 and I authorize for medical care to be administered as required by a trained medical professional. I agree to release KIDS PARADISE from all claims, damages, injuries and expenses arising out of such assistance, including any claims arising from contact needed to administer assistance.

§ A waiver signed by the minor’s parent or legal guardian, is required for each participant before entering Play Area.

§ Parent(s) or Adult (age 18+) Supervisor MUST accompany child(ren) inside of Play Area at all times. (no siblings 10+ years can’t assist)

§ No shoes allowed. Socks are required for EVERYONE in Kids Paradise.

§ No outside food or drinks allowed.

§ Absolutely no food or drinks allowed in play area.

§ No sharp objects or toys allowed in the play area.

§ No horseplay or roughhousing.

§ Respect others around you.

§ Parents of 2 years & under MUST accompany their child at ALL times.

§ No drops-off allowed.

§ 1 Warning – if anyone from the Kids Paradise staff sees children playing in a way that is dangerous, we will ask them to stop and do our best to locate the parent/guardian to ask them to help us control the child’s behavior. After 1 attempt to control the behavior of the child that might cause harm to his/herself or to other children, we may ask you to leave for the day and your entrance fee is non-refundable.

§ Sick Child Policy. Please refrain from bringing in a child with any type of contagious illness. After all this is a children indoor playground and we always o keep our environment as healthy as possible.

§ We are not responsible if your child leaves the premises

§ We STRONGLY REQUEST ALL THE PARENTS TO MONITOR YOUR OWN KIDS AT ALL TIME. KIDS PARADISE IS NOT RESPONSIBLE FOR ANY CHILD MOLESTIC AND CHILD SEXUAL ABUSE, COMPLAINTS AND CLAIMS BY OTHERS.
