In order for Elevated Sound to properly prepare for your Event and have the necessary tools and
props on hand, it is imperative that you fill out and return this Required Information Form no later
than thirty days prior to the Event.
Please email the completed form to can provide a certificate of insurance in advance if requested, if not we will supply the certificate on the day of the event.
