SYNOVIAL FLUIDS FOR CRYSTALLOGRAPHY

Since November 2013 when the Histology laboratory at East Cheshire NHS Trust was relocated to Leighton hospital synovial fluid specimens requiring crystallography studies have been sent directly to Central Manchester Cytology Centre for examination.

This change has meant that only the central Manchester request form needs to be filled in and not the old East Cheshire non gynae request form.

Recently there have been a number of occasions when this has not happened, causing delays in the specimens being processed. So, this is just a gentle reminder;

Please only use the Central Manchester Cytology request form for synovial fluid specimens requiring crystallography

The form can be downloaded from the link below;

Send the specimens with the completed request form to Specimen Reception, Pathology, East Cheshire NHS Trust for transportation on to Manchester. Results will be sent directly to the requesting clinician from Manchester.

CONSENT FORM 2

CONSENT FOR THE EXAMINATION OF TISSUE FOLLOWING MISCARRIAGE - FOR POC LESS THAN 12 WEEKS GESTATION

Following a request from the Lead Consultant Histopathologist, it has been decided that due to new policies regarding consent and patient wishes for disposal, introduced this April, we will no longer accept any old consent forms into the laboratory from 18th August 2014. The new form has been in place for some time now to take into account the new policies, but we are still receiving old request forms into the laboratory with specimens. The new form is available on the Intranet under ‘frequently used forms’ at MCHFT and has kindly been circulated at MCHFT and ECT, but there seems to be some old forms which have not been destroyed.

Our procedure, should we receive any of the old forms after 18th August 2014 will be to return the form to the originating ward/dept, so that the appropriate updated form can be completed. We understand that this may cause distress to patients whom have already completed a form, but we must ensure that policies relating to specimen disposal and consent are adhered to.
