Dear Referring Partners:
We are excited to introduce our new Clinical Nurse Manager for our Access Center her name is Jill Hoomes. She will be available to assist with scheduling needs of our patients along with Brenda Montour.
We have reviewed our current process of receiving referrals and have determined that a revision of the form was needed. We are requesting that previous referral forms are discarded and exclusively use this revised version. We have posted the newly revised referral form and this notification on the Western Nephrology website.
The reason for this revision is to best care for our mutual patients and to provide a more seamless process for all. The development of new blood thinners have provided potential risk of bleeding to patients when they are undergoing access procedures. Currently, we ask that patients stop Coumadin 3 days prior to the procedure; this order must be obtained from the physician who prescribed this medication to the patient. We need to include the following medications: Eliquis, Pradaxa, and Xarelto be stopped 2 days prior to the procedure. An order to stop these medications must also be obtained from the physician who has prescribed the medication to the patient.
We are requesting when a referral is made to the Access Center the below information is sent:
1. Referral form filled out completely
2. Medication List attached
3. Current Patient History, if available
