ADDENDUM #1

To: All Planholders for Invitation for Bid Entitled “Paine Black House and Burgess House Roof Replacement Project” dated October 7, 2014

Due Date: November 4, 2014, 2:00 pm

Please be advised that Addendum #1 is being issued to the subject Invitation for Bid for the following reason:

The following items shall modify or be added to the Bid Documents and Project Manual. This

addendum forms part of the Contract Documents and modifies the original bidding documents.

Portions of the Bidding and Contract Documents not altered by this addendum remain in full force.

These addendum form part of the Contract Documents and as such,

ALL BIDDERS SHOUL ACKNOWLEDGE IT IN SECTION B ON THE FORM FOR GENERAL BID. FAILURE TO DO SO COULD RESULT IN THE REJECTION OF YOUR BID.

Changes are as noted below.

1. GENERAL:

Drawing A1: Paine Black House.

o Red Cedar roof shingle exposure is shown on the drawings as 10". Roof shingle exposure should read 6" maximum.

o The plans call for new 5/8" plywood roof sheathing to be installed over the existing wood roof plank deck. It has been discovered that strand board roof sheathing exists under the asphalt shingle roof. Therefore, the 5/8” plywood and its installation over the entire roof surface is deleted. The strand board is to remain as the roof sheathing. The contractor shall carry 128 square feet of new 5/8” plywood sheathing to replace any rotted existing roof sheathing.

o The plans call for new rakes, rake trim, and fascia to be installed. This work has already been completed by the Town Of Barnstable. No roof rakes, rake trim, fascia work is included in this project for the Paine Black House.

o The plans call for new PVC downspouts. Change PVC to 3" painted galvanized steel pipe down spouts for the Paine Black House work.

o All through roof plumbing penetrations shall have new flashing.

o A new drawing A1 is provided with this addenda to replace the previous drawing A1

2. Specification:

o Section 07317-1 Wood Shingles and Shakes, Paragraph 1.1 item C: Add:

Existing roof sheathing shall remain on the Paine Black House. Carry 128 square feet of replacement sheathing for this building in addition to 50 LF of replacement roof deck boards.

Paragraph Part 3 - execution sub paragraph 4 b. Change exposure to 6" max.

o Section 07710-1 Manufactured Roof Specialties, 2.2 Paragraph B, 2

Delete entire paragraph titled Downspouts, and replace with:

Downspouts:

Burgess House: Round PVC, with metal goosenecks. Style to match existing. Furnish wall brackets to match profile of existing brackets with anchors.

Paine Black House: Painted 3" round galvanized metal downspouts with gooseneck attachment to new wood gutter. Furnish wall brackets and anchors as approved by the architect.

o Section 09910-1 Part 4.1 A: Add:

Extent of painting work includes all new and disturbed wood installation. For partial element replacement, include painting of entire element for consistent appearance. Painting of replaced shingles shall be to match existing adjacent painted shingles only.

3. Section 5, FORM FOR GENERAL BID - has been revised to add the 128 square feet of new plywood sheathing to replace any rotted existing roof sheathing for the Paine Black House portion of the project.

Please acknowledge Addendum 1 on your bid submittal Form. All other terms and conditions remain the same as previously stated.

The contractor shall carry 128 square feet of new 5/8” plywood sheathing to replace any rotted existing roof sheathing on the Paine Black House. The contract shall provide a unit price per square foot and shall be paid for the actual amount of sheathing required for this project. Price carried in above pricing unit C shall include the 128 Square feet.

Unit pricing quoted as part of this project is:

Rule for award: Award will be made to the lowest responsible eligible bidder for the base bid plus selected alternates. The Town reserves the right to reject all bids if deemed in its best interest.

E. The undersigned agrees that, if he is selected as general contractor, he will within five (5) days, Saturdays, Sundays and legal holidays excluded, after presentation thereof by the awarding authority, execute a contract in accordance with the terms of this bid and furnish a performance bond and also a labor and materials payments bond, each of a surety company qualified to do business under the laws of the Commonwealth and satisfactory to the awarding authority and each in the sum of the contract price, the premiums for which are to be paid by the general contractor and are included in the contract price; provided, however, that there is more than 1 surety company, the surety companies shall be jointly and severally liable.

The undersigned hereby certifies that he is able to furnish labor that can work in harmony with all other elements of labor employed or to be employed on the work; that all employees to be employed at the worksite will have successfully completed a course in construction safety and health approved by the United states occupational Safety and Health Administration that is at least 10 hours in duration at the time the employee begins work and who shall furnish documentation of successful completion of said course with the first certified payroll report for each employee; and that he will comply fully with all laws and regulations applicable to awards made subject to Section 44A of Chapter 149 of the Massachusetts General Laws.

The undersigned further certifies under the penalties of perjury that this bid is in all respects bona fide, fair and made without collusion or fraud with any other person. As used in this subsection the word “person” shall mean any natural person, joint venture, partnership, corporation or other business or legal entity. The undersigned further certifies under penalty of perjury that the said undersigned is not presently debarred from doing public construction work in the commonwealth under the provisions of section twenty nine F of chapter twenty-nine, or any other applicable debarment provisions of any other chapter of the General Laws or any rule or regulation promulgated hereunder
