To: Parents of Children on ADD/ADHD medications

Regarding: ADDERALL, ADDERALL XR, CONCERTA, DAYTRANA, FOCALIN, METADATE CD, RITALIN, RITALIN LA, RITALIN SR, VYVANSE, INTUNIV and STRATTERA prescriptions

Children taking any of these drugs MUST have yearly physicals and every three- month follow up visits to receive refills. A three- month med check is NOT a physical and needs to be scheduled in advance to insure uninterrupted refills. Please note this on your calendar and call for an appointment a month in advance.

If a change in dose is required, you must first speak with a physician.

You may call for refills Monday through Friday during daytime business hours only. Again, we suggest you mark your calendar so you are not caught unaware that you are running low. No prescriptions will be written on Saturdays!

We require 24 hours notice for these prescriptions.

The prescription may be picked up at the office or mailed to you. We CANNOT call the prescription in to the pharmacy (except for Strattera and Intuniv), as these are controlled substance medications. We will not fax or mail directly to mail-in pharmacies. We have had many problems in doing so, therefore we will mail script to you or it can be picked up for you to mail to your mail-in pharmacy.

The prescription must be filled within 60 days of date written.
