`Title I Schoolwide Program Annual Review Scoring Rubric Date:
District Name: Schools Name: Score:
To continue to implement a schoolwide program, the plan must meet the minimum requirements for each of the sections below. Schools plans with one or more requirements scoring less than 4 points must submit approval documentation before approval will be granted. The district can choose to re-submit those portions or to notify the Title IA Office at 624-6705 of your decision to withdraw the application for this school year. For schools that receive a required minimum of 70 out of 96 possible points, a schoolwide project sheet will be added to the Title IA portion of the Consolidated No Child Left Behind (NCLB) Application. Please note, however, that the Title IA portion of the Consolidated NCLB Application cannot be approved until the schoolwide annual review has also been approved.
