2013 OPEN ENROLLMENT

I am pleased to announce the 2013 Benefits Open Enrollment period for Xavier University of Louisiana will begin December 3, 2012, and end on December 7, 2012. All changes made during Open Enrollment will be effective January 1, 2013. The Open Enrollment period is your opportunity to review your current benefit elections to ensure that they continue to meet the needs of you and your family. Plan representatives will also be available during open enrollment to answer your questions or to provide you with information.

Xavier continues to be committed to offering a comprehensive, cost efficient and competitive benefits package to all eligible faculty and staff by continuing to be aware of developing trends and opportunities in the benefits marketplace. Humana will continue to administer Xavier’s health, dental and vision plans and the university will continue to offer the Transamerica Cancer and the Ceridian Flexible Spending Plans.

The University will continue to pay a major portion of your healthcare premiums, however the university’s contribution will be reduced due to the significant challenges the university is facing. Even though the university’s contributions to your health insurance premiums for 2013 will be reduced they are still considerably more than employers which offer similar employee benefits. Please see the attached premiums for all plans for 2013, and the Open Enrollment meeting schedule.

Xavier has added two new health insurance plans Humana High Deductible and Ochsner Total Care HMO in addition to the Enhanced and Base plans which will give you a total of four options for which to choose to meet the needs of you and your family at an affordable cost. These plans will be discussed in detail at our open enrollment meetings and representatives from Humana and Ochsner will be available to provide you with additional information and to answer your questions.

All enrolled employees and their dependents must work to do their part in helping Xavier to control the cost of insurance premiums by daily exercise and eating healthier foods. This fall, Humana offered their Vitality Program to its health insurance members to measure factors affecting health and to provide rewards for achieving health goals.

During the Open Enrollment period you will be required to do one or more of the following:

· You will be required to complete the attached “Employee Confirmation of No Change in Coverage” form if you are not making any changes to your health, dental or vision coverage and you must return the completed form to the Office of Human Resources by December 7, 2012. Please feel free to fax the form to 520-7937 or email it to .

· If you are enrolling or making changes to your health, dental and/or vision plans you will be required to complete a new enrollment application for 2013. An application must be completed by all employees wishing to enroll in the Flexible Spending and Transamerica Cancer Plans. All applications must be received in the Human Resources Department on or before December 7, 2012.

All enrollment applications must be received in the Office of Human Resources on or before December 7, 2012. No enrollment applications will be accepted after this date.

I understand and agree to the following:

· My medical, dental and/or vision plan coverage will continue at the rates that are effective January 1, 2013.

· My medical plan choice is locked in for the full plan year (1/1/2013 – 12/31/2013).

· I will not have the opportunity to change medical plans until the 2013 open enrollment period unless I have a life changing event (marriage, divorce, birth of a child, cessation of a spouse’s coverage or death of a covered spouse or dependent).
