INSTRUCTIONS FOR COMPLETING THIS APPLICATION

1. All sections of this application must be completed! Attach additional sheets of paper if you need more space. Please verify that all necessary signatures (applicant, parent or guardian and high school counselor) are included on the application. You may submit a copy of the application form.

2. An official copy of high school transcript and SAT/ACT scores must be included with the application. Feel free to provide any additional information you desire.

3.The application and all supporting documentation must be received by the HCCA no later than Monday, March 24, 2015. Applications received after that date will not be considered.

4. Applicants may be asked to appear before the scholarship trustees for an interview.

5. Scholarship winners will be notified in writing by HCCA no later than May 1, 2015.

GENERAL INFORMATION

AMOUNT OF ANNUAL GRANT
The Sidney R. Johnston Memorial Scholarship Trust will award two scholarships in 2015. Each Scholarship is worth $10,000.00, and will be paid on the basis of $2,500.00 for each academic year of a four-year college program (provided the recipient remains academically eligible throughout his/her time in college).

ELIGIBILITY
Any college-bound high school senior who will be enrolling full-time in an academic institution as of September of 2015 is eligible to submit an application.

Applicants must be between the ages of 17 and 24, and be currently enrolled as a senior in high school or be a high school graduate considering enrolling as a freshman to a four-year college or university.

Applicant or applicant’s family member must have service or involvement with a firm that is a member of the Heavy Construction Contractors Association.

CRITERIA USED TO SELECT SCHOLARSHIP WINNERS
The successful scholarship applicant will be judged on a number of points, including but are not limited to:

1) grades in high school ;
2) national college board scores (SAT, ACT);
3) participation in high school & community extra-curricular activities;
4) contributions to the individual’s high school and /or community;
5) the candidate’s positive character, morality, and motivation to succeed;
6) financial need.

The Scholarship program is open to all eligible applicants. Employment or affiliation with a firm that is a member of the Heavy Construction Contractors Association is a requirement for consideration of the scholarship. .

WINNERS: Must provide social security number as well as information needed to process checks to educational institution.

GOVERNING BODY FOR THE SCHOLARSHIP

The Scholarship Board of Trustees is a non-profit, IRS 501C(3), charitable organization affiliated with the Heavy Construction Contractors Association. The Trustees administer the selection process and maintain continuity over the administration of the scholarship program.
