SDDC Annual Awards - Nomination Guidelines
Below is the form to help facilitate nomination of an organization for one of the two award categories. Coalition members can nominate any firm (including themselves) or individual for the awards outlined below. A letter of recommendation explaining why the firm should be considered must be submitted to the Awards and Special Events.

Diverse Business of the Year
Award Criteria:
Nominees for MWBE of the year can be any certified M/W/D/V/BE operating in the State of Indiana. Coalition members or Chamber of Commerce members may nominate one business in each diverse classification (M/W/D/V/BE) in accordance with the following criteria. A detailed one-page letter explaining why the firm you are nominating should be awarded Diverse Business of the year is required with submission of this nomination.
The following three criteria must be submitted or obtained from the diverse firm. Please limit each submission to one page.
· Must be operating in the State of Indiana
· Must be certified by a credible third party
o Any government agency certification (SBA, IMSDC, WBENC, etc.)
· Planning for the future
o MWBE under consideration must submit a brief statement (not more than one page) describing future business plans.
o Projected Growth (i.e.: additional staff, additional employees, higher credit limits, bonding, etc.)
o Social Responsibility (i.e.: green initiatives, LEED certified, mentoring, etc.)
