RELATIONSHIPS
1. Responsible To: Director of Public Health
2. Responsible For: delivery of community evidence based bone health exercise interventions for those aged 45 and over in the borough of Kingston.
3. Important Functional Relationships: better bones team, Get Active Team, Adult Social Care , housing and care teams.
4. Important External Relationships: Kingston Hospital, Kingston Falls Service, Fuel Poverty Services, Voluntary Sector Organisations, Leisure centres, GP’s.

MAIN PURPOSE OF JOB
1. Provide group exercise programmes for people with bone health conditions including osteoporosis and osteoarthritis.
2. Delivery of evidence based weight bearing exercises, balance exercises and educational classes for clients attending the 14 week programmes
3. Increasing exercise adherence, confidence levels and bone health knowledge of older people with bone health diagnosis or risk factors in Kingston
MAIN RESPONSIBILITIES/DUTIES OF JOB
• Provide group exercise programmes for people with bone health conditions including osteoporosis and osteoarthritis.
• Setting up and delivering 1 hour exercise classes within the community in Kingston across different venues.
• Delivery of weight bearing exercises, balance exercises and educational classes for clients attending the 14 week programmes.
• Ensuring the safety of all clients while delivering the exercise classes.
• Reporting back any concerns to the service manager.
• Managing attendance of classes including paperwork submission and contacting clients who have not attended.
• Provide osteoporosis screenings to people 50+ years old in Kingston in community settings, where needed.
• Hold education sessions to service users on bone health, exercise and nutrition.
• Represent the Bone Health Service at community events.
• Keep service database up-to-date.
• Maintain data collection systems to ensure the progress and outcomes of the project can be monitored adequately.
Communication:
• The post holder will be required to liaise with a broad range of health care professionals, third party organisations and internal and external stakeholders; including the Kingston Falls Service, GPs, Kingston Hospital, sheltered housing schemes and care homes.
• Support the work of the Bone Health Specialist to develop and implement project data collection systems that will provide accurate and timely data.
• Report regularly on issues, referral numbers and service uptake to the service manager.
• Support the work of the Bone Health and exercise specialist.
• Refer patients onto other exercise interventions when appropriate.
Training and Development:
● Update CPD, professional knowledge and skills in line with current developments and guidelines in relation to the delivery of bone health exercise programmes.
Organisational Responsibilities:
● Have a flexible approach to working hours in order to meet the needs of the service.
● Support the implementation of the Service Specification through timely and relevant information analysis and administrative support, in accordance with agreed priorities of the team.
● Deliver against agreed objectives, achieving quality outcomes.
● Actively support and contribute to the development of key performance indicators for successful assessment of performance.
