Registration Process for 2014-2015 School Year

New KG (5 Yrs Old) and PS4 (4 Yrs Old) Students

Must Be Age Appropriate On or Before July 31, 2014

1.  Verify the proof of address and immunization and birth certificate documents required for registration

·  An additional piece is required if a Driver’s License is presented as this document does not require verification by the Dept. of Revenue to be issued

2.  Parent/Guardian should complete the Student Registration form.

·  PLEASE make certain that all information is completed where possible.

The questions with boxes below the bold line and type in the middle of the page must be answered under District and DESE guidelines when registering a student.

3.  School staff should complete the Student Assignment form, pulling information from the SIS database for students re-entering the district.

·  Students returning from last school year do not need to register, only students who did not finish 2013-2014 in an SLPS school

Take the steps necessary to:

A.  Determine if the student is supposed to attend your school by logging in to the

E4CO-1415 or your schools 2014-15 fileset in SIS, then use

to lookup the address and block number to learn the correct assigned school to be entered on the Student Assignment form

·  Students may be assigned to schools not in the assigned boundary with approval of Principal

·  NO BUS TRANSPORTATION WILL BE PROVIDED TO STUDENTS ENROLLED IN SCHOOLS NOT IN THEIR ASSIGNED BOUNDARIES WITHOUT THE APPROVAL OF THE TRANSPORTATION DEPT.

·  Newly accepted magnet and choice schools students must have an acceptance letter from the Student Recruitment & Placement Office (Formerly Recruitment & Counseling) to gain admittance to these schools IF the student is not listed on acceptance rosters or in the school SIS database. These letters can vary to include:

* Documents typed on letterhead

* Handwritten memos

* Emails form the Magnet/Choice Schools Application System.

B.  Determine if the student has been in the SLPS system by searching for a student number via logging in to the E4CO-1415 fileset in SIS, then click

Student-User Defined Data

·  Enter the last name of the student

·  Enter the first 2 or 3 letters of the first name

·  Click in OR

to find Student ID# using birth date to weed out students with the same name(s)

*DO NOT ENROLL THE STUDENT IN THE E4CO FILESET*

New and re-entry students must be enrolled into the individual school fileset.

4.  Click tab and scroll down to to determine special education

needs/services the district may have provided if previously enrolled in SLPS. Please note the 2 digit code(s) on the Student Assignment form.

5.  Upon completing these steps…

·  Fill out the remainder of the Student Assignment form using the information provided (i.e. birth certificate #) or completed by parents on the Student Registration form.

·  Check what may be on the SIS database such as last SLPS school attended, and withdrawal code by checking the circle on the in SIS.

·  Ask parents the questions that may be need answers to complete the forms entirely and enroll the student into your schools SIS fileset.

6.  Now enroll into your individual Schools Fileset UNLESS…..

IMPORTANT ITEMS OF NOTE

·  For Student Registration forms answering YES to questions about using a language other than English, parents should be referred to the ESOL Offices, Maria Childress.

·  For Student Registration forms answering yes to questions about the Safe Schools Act, parents need to be referred to the Innovative Pathways Offices at 801 No. 11th St. Dr. Karen Cason.

·  For Student Registration forms stating the residence is anywhere other than “In permanent, stable housing with parent(s)”, the McKinney-Vento Act questions on the Student Assignment form must be asked.

A copy of the Student Registration form must be sent to the Students in Transition (SIT/Homeless) Office, Deidre Thomas-Murray.

Parents are not required to be referred to the SIT offices but more services are available for the families if they do.

7.  Finally, yellow carbons/photocopies of both the completed and signed Student Registration and Student Assignment Forms should be pony mailed to the Student Recruitment & Placement Office, with the top original white copies kept on file at the school.
