This event has a limited amount of Booth/Exhibit space. Space will be assigned on a first come first serve basis!

Time: 9:00 AM to 2:00 PM
Load in: 7:00 to 8:00 AM
Fire Department Inspection: 8:30 AM Sharp
Deadline for Application: July 20th, 2015
Ready Set...Phoenix Kids Get Fit is focused on fitness, health and nutritional services for families getting ready for school. There will be dental, vision, and hearing checkups as well as free immunization's and many fitness activities. Our goal is to provide resources that encourage families to lead a healthy active lifestyle.
We request that all booths/exhibitors provide an interactive game or hands-on activity related to the wellness theme. We encourage educational games, physical activates, and/or souvenirs that promote a healthy lifestyle. This type of interaction gives the participant the opportunity to share their experience with family and friends!

Checks, money orders or cashier checks are payable to the Latino Institute and
must be received with this completed and signed application. Business and
personal checks written on US banks will be accepted provided: Personal checks
require a phone number and address printed on the check. Post dated and/or
third party checks are not accepted. There is a $40 charge for all returned
checks No refunds after July 17th.
All spaces are issued on a first come first serve basis. You will receive a full
refund if not selected. The City of Phoenix Latino Institute determines the final
selection of applicants. Applications are due by July 17th, 2015 walk in/mail

Vendor agrees to indemnify, defend, save and hold harmless the Latino Institute/City of
Phoenix and its officers, officials, agents, and employees (hereinafter referred to as
“Indemnitee”) from and against any and all claims, actions, liabilities, damages, losses,
or expenses (including court costs, attorneys’ fees, and costs of claim processing,
investigation and litigation) (hereinafter referred to as “Claims”) for bodily injury or
personal injury (including death), or loss or damage to tangible or intangible property
caused, or alleged to be caused, in whole or in part, by the negligent or willful acts or
omissions of Vendor or any of its owners, officers, directors, agents, employees or
subcontractors. This indemnity includes any claim or amount arising out of or recovered
under the Workers’ Compensation Law or arising out of the failure of such vendor to
conform to any federal, state or local law, statute, ordinance, rule, regulation or court
decree. It is the specific intention of the parties that the Indemnitee shall, in all
instances, except for Claims arising solely from the negligent or willful acts or omissions
of the Indemnitee, be indemnified by Vendor from and against any and all claims. It is
agreed that Vendor will be responsible for primary loss investigation, defense and
judgment costs where this indemnification is applicable. In consideration of the award of
vending rights, the Vendor agrees to waive all rights of subrogation against the City, its
officers, officials, agents and employees for losses arising from the vending performed
by the Vendor at Ready Set...Phoenix Kids GET FIT! August 1, 2015
Vendor has read, understands and agrees to comply with all event regulations. Vendor
understands failure to abide these rules could result in the probation and/or exclusion
from all city of Phoenix produced events.
