INSTRUCTIONS TO SUPERVISOR:

1.  Please complete this form completely and carefully. It is to help you:

a.  Be objective in doing performance appraisals for all your employees.

b.  Look at each of the individual behavior related to employee performance.

c.  Support your conclusions and recommendations for areas of improvement, disciplinary action, or any other actions.

2.  PLEASE USE THE EMPLOYEE’S JOB DESCRIPTION HAND-IN-HAND WITH THIS TOOL. Ten components related to overall assessment of job performance are listed. For each one, decide the level at which the employee performed for this appraisal period. Place a check the category that best describes the employee’s performance, using the scale below:

Superior: Performance consistently far exceeds established standards and expectations. Requires

absolute minimum or no direction.

Commendable: Performance usually exceeds established standards and expectations. Minimal

direction is required.

Satisfactory: Performance consistently meets established standards and expectations. Moderate

direction is required.

Marginal: Performance sometimes meets established standards and expectations but not on a

consistent basis.

Unacceptable: performance is consistently below established standards and expectations.

frequent direction is required. Substantial improvement is necessary to maintain

employment.

3.  Comment in writing on the employee’s major strengths and areas for growth and make suggestions for improvement. Your comments should include specific incidents and examples that relate to your ratings.

4.  Discuss the appraisal with the employee and encourage the employee to add comments to the form. Agreements with the employee should be worked out at this point and put in writing.

5. The employee and you should then sign the form acknowledging completion of the appraisal and actions to be taken.
