Agreement
Daycare Service
We agree to provide daycare services to your dog and give the dog medication as directed on the Medication Form. Our daycare hours of operation are from 7:00 a.m. – 7:00 p.m. Monday thru Friday and Saturday 9:00 a.m. – 5:00 p.m. We will take care of your dog for any amount of time during these periods. Our staff will exercise careful, reasonable judgment in all situations during your dog’s stay with us. All dogs must be Temperament Tested before being admitted into daycare. The Temperament Test must be scheduled in advance.
Payment for Daycare/Sleepovers
You agree to pay GoodFellas Doggy Daycare the scheduled fee before your dog is taken into our care. These fees are provided for you on our fee schedule. Your dog’s sleepover includes a full day of daycare for each night stayed. You may pick up your dog anytime during daycare hours. Please call for accommodations that vary from our regular schedule. Guests left beyond our normal daycare hours without prior notice will be charged an additional night’s stay.
Reservations
Reservations are required for dog sleepovers but not for daycare. Payment for sleepovers must be made at the time your dog is dropped off. Please cancel your reservations at least 24 hours in advance if your plans change.
Emergency Procedures
GoodFellas has an emergency evacuation plan in place. GoodFellas Staff is also trained in Pet First Aid & CPR but, in the event of an emergency, every attempt will be made to contact Parent 1, Parent 2 and the Emergency Contacts listed on the GoodFellas Doggy Daycare Enrollment Form. If contact cannot be made, then we will act in a way that we feel is in the best interest of your dog which includes providing professional veterinary services. You agree to not hold GoodFellas Doggy Daycare and its staff liable for any decisions we make. You shall be responsible for all costs incurred with the care of your dog which includes, but is not limited to, emergency veterinary care. GoodFellas recognizes Massachusetts Veterinary Referral Hospital (IVG), 20 Cabot Road, Woburn, MA 01801 for 24 hours emergency care.

Dog Health & Behavior
No dog can stay with GoodFellas unless they have proof of the required vaccinations by a licensed veterinarian. All required vaccinations are listed on the GoodFellas Doggy Daycare Enrollment Form. If your dog is found to have fleas or ticks, then you agree to allow us to remove and treat at your expense. We also reserve the right to not allow your dog into our facility if your dog looks sick, injured, or if we feel your dog requires professional medical care. If your dog becomes sick while in our care, we will attempt to contact Parent 1, Parent 2 and the Emergency Contacts listed on the GoodFellas Doggy Daycare Enrollment Form. If contact cannot be made, then you agree to allow us to act in the best interest of your dog. Dog owners are responsible for all costs incurred with the medical care of their dog. GoodFellas Doggy Daycare recognizes VCA Animal Hospital, 19 Main Street, Wakefield, MA 01880 for all non-emergency care.
Your Representations To Us
You represent that you are the owner of this dog, at least 18 years of age, are authorized to enter into this agreement and you agree that all information provided is true and correct to the best of your knowledge. You also agree, to the best of your knowledge, that your dog has not been exposed to rabies, distemper or any other contagious illnesses within 30 days prior to beginning its stay with us. You also agree that your dog has no illnesses or behavior problems that have not been disclosed to us and that you will hold our company and its employees harmless for any loss, damage or expense resulting from your dog’s stay. You shall also hold us harmless from, and indemnify us against, any claims asserted by any person who was either injured or harmed by your dog and, in addition, you shall hold us harmless from any injuries sustained by you or your dog. This disclaimer shall be interpreted to the broadest extent possible under Massachusetts Law. You also acknowledge that in the unlikely event that your dog is injured by another dog, you will not hold us responsible for the injury. If your dog injures another dog, you will be solely responsible for the injury. You further agree that you will not hold us liable if your dog becomes ill during or after it’s stay at our daycare or overnight facility.
Dogs Not Picked Up
If your dog is not picked up during normal daycare hours, you agree to allow us to continue housing and boarding your dog at your expense. If your dog is abandoned as defined by Massachusetts Local or State Laws or by our Abandoned Pet Procedure, we will make every attempt to contact Parent 1, Parent 2, the Emergency Contacts and your listed veterinarian. We will also send a certified letter to the address given on the Enrollment Form before further action is taken. If your dog is not picked up within 10 days of the expected pick up date, your dog may be given up for adoption or processed through a third party adoption center.

This agreement can be changed only if in writing and signed by you and our company.
You acknowledge that GoodFellas Doggy Daycare reserves the right to refuse admittance to your dog at any time if problems arise from your dog’s behavior.
You further agree (for yourself and all others acting on your behalf) that by allowing your dog to participate in services offered by GoodFellas Doggy Daycare you consent to our taking of photographs and videos of both you and your dog, and you consent to our use of said images and videos (in print form or otherwise) of you and your dog, for our company’s publication and/or promotion.
