Infant Rollover Permission

Oakridge Early Education Center’ policy requires an infant be placed to sleep on his/her back. If a child under six months of age repositions him/herself during sleep, Maryland State licensing regulations requires a signed parent/guardian permission to allow the child to remain on his/her stomach.

I, the parent/guardian of _, give permission for my child to remain sleeping on his/her stomach after rolling over during sleep.

I, the undersigned, being the custodial parent/legal guardian of (child’s name)

, a minor, on behalf of myself and said minor hereby release and agree to hold harmless, defend and indemnify Oakridge Early Education Center, its subsidiaries, and employees from any and all claims of any nature, including claims of personal injury, resulting from my direction set forth above.
