NOMINATION FOR THE MOST INNOVATIVE POLICE-YOUTH PROGRAM AWARD

To recognize the police officer who created the most innovative police-youth program which was funded by ProAction and took place in 2016 across the City of Hamilton.

Eligibility: 1). Innovativeness of the program, what makes it so unique;

2). Time and effort spent by the officer;

3). Impact of the officer on police-youth relationships.

Nomination: Police officers are asked to forward their recommendations to ProAction

no later than January 27th, 2017. Recommendations for this award shall

be reviewed by the Hamilton Chapter Council in consultation with the

Hamilton Police Liaison.

Presentation: This award shall be presented at the Annual Meeting of the ProAction

Cops & Kids Board of Directors.

Requirements: 1). Limit each answer to 250 words or less.

2). Complete the form to its entirety, making sure it meets criteria.

3). Recipient must be a police officer.

4). Please ensure the program started, falls or runs in the year

being considered.
