Newport Aquarium Field Trip Permission Form

Mrs. Hess’ Marine Sciences class will travel to the Newport Aquarium on Thursday, Nov. 13, 2014 at 8:15 am and return at the end of the day. The cost of the trip is $10.00. Additional money will be needed for lunch, snacks, and gift shop. We will have a snack opportunity in the aquarium and stop for a late lunch on the way back to school.

*****Permission slips must be returned with money, med forms, and all signatures ASAP. Due to processing of funds, I have to turn all paperwork in at one time. This date will be posted on the board. Hand these forms TO ME! DO NOT PUT IN MY MAILBOX OR UNDER DOOR!!! Cash or Checks accepted.

Checks may be made to: Beavercreek City Schools but cash is preferred for ease of payment and ease of returning funds if student is absent. Will be discussed with students.
